My Health Connection: your online patient portal

We encourage all our patients to sign up for My Health Connection. This is your personal patient portal to request prescription refills, view your medical records or test results and to contact our office.

At UCHealth Primary Care – Arvada West, we offer the best in primary care and sports medicine services. Our highly-trained and compassionate staff can help you obtain and maintain optimal health.

From routine physicals to behavioral health services to maintenance of chronic conditions like diabetes or cholesterol management, our providers take the time to listen to your concerns and to develop a plan for care with you.

Primary care services

Behavioral health

UCHealth offers behavioral health services to established patients at this clinic. During a visit with your primary care provider, please bring up any mental, emotional, health behavior or life stressors you may have. Your provider can refer you to a behavioral health clinician.

What you can expect:

  • UCHealth behavioral health providers and clinicians can help with a wide range of needs, which could include: stress, anxiety, depression, sleep, substance use, chronic illness, grief, pain, weight loss, smoking cessation, parenting, relationships, and much more.
  • The behavioral health team can help coordinate referrals to higher levels of care for more acute needs. Including, but not limited to: UCHealth outpatient clinics, our Virtual Intensive Outpatient Services or Interventional Psychiatry at UCHealth.
  • Behavioral health visits may be covered under mental health benefits. For insurance-specific information, please reach out to your insurer.

If you are experiencing a mental health crisis, please call 988, 911, or go to your local emergency room.

Your appointment

What to bring

  • Your photo ID (driver’s license, passport, state or military ID are acceptable).
  • Your insurance card. If covered by a spouse’s policy, know your spouse’s date of birth and Social Security number.
  • List of current medications taken.

Billing and Insurance

UCHealth participates with many different insurance companies. Since insurance plans, policies and laws often change, please contact your insurance company to see if we are a contracted provider.

Please notify us if you have an insurance plan change, coverage change or insurance loss, so we can properly bill your claims in a timely manner.
