Primary care and specialty care in a patient-focused design.
Our family medicine practice follows a team-based care design. It has four “pods,” or centralized areas. Each pod can accommodate two or more physicians, five MAs, staff nurses, a care manager or social worker, a psychologist, a “Primary Care Redesign” coach, a “Patient Centered Medical Home” coach and a pharmacist.
The floor includes many upgrades, including separate space for echocardiograms and cardiac stress testing; an expanded dedicated area for the center’s anticoagulation nurse; larger lab facilities; and a radiology suite with a dedicated waiting area.
Our rehabilitation space includes new equipment for Pilates, parallel bars for neuro and amputee patients. Its airy, open design encourages collaboration between therapists.
Rehabilitation appointments: 7 a.m. to 5:45 p.m.
