The primary goal of cardiac or pulmonary rehabilitation is to enable participants to achieve optimal physical, psychological and social functioning through exercise training and lifestyle change.
Our cardiopulmonary rehabilitation team provides individualized and personalized supervised and monitored exercise, chronic disease and risk factor education, nutrition counseling and mental well-being assessment and counseling for persons with heart or lung diseases.
Our multidisciplinary team is made up of exercise physiologists, registered nurses, registered dietitians and licensed social workers who coordinate care with cardiologists, pulmonologists and primary care physicians. We keep our participants and families at the center of their treatment plan for lifestyle changes and a goal-driven recovery.
