What to bring
- Your photo ID (driver’s license, passport, state or military ID are acceptable)
- Your insurance card. If covered by a spouse’s policy, know your spouse’s date of birth and Social Security number
- List of current medications taken by your child
My Health Connection - your online patient portal
We encourage all of our patients to sign up for My Health Connection and download the UCHealth mobile app. This is your personal patient portal to request prescription refills, to view your medical records or test results.
Child access. My Health Connection’s child access can be used by parents or legal guardians of dependent children. Access by parents or guardians is not restricted for patients under the age of 14. Patients aged 14 to 18 may have the legal right to consent to certain types of medical treatment themselves. They do not need consent by their parent or guardian. UCHealth respects the rights of these young patients to keep their medical care private.
