My Health Connection: your online patient portal
We encourage all our patients to sign up for My Health Connection. This is your personal patient portal to request prescription refills, view your medical records or test results and to contact our office.

The UCHealth Longs Peak Hospital Pharmacy is open to the public 24 hours a day, seven days a week, with a clinical pharmacist available at all times. We are the only 24-hour pharmacy in Longmont and Boulder County.
We’re located on the first floor of UCHealth Longs Peak Hospital, next to the gift shop—after hours, you must enter through the emergency department. Our inpatient and retail pharmacists—as well as support staff—are cross-trained to work together so you can expect expedient, friendly service and counseling from everyone.
We stock over-the-counter medications, crutches, canes and walkers.
We offer 24-hour prescription refills.
Our Meds to Beds program allows your doctor to send prescriptions down to the pharmacy prior to your discharge from the hospital. We fill the prescription(s) and deliver the medication(s) to your bedside, eliminating an extra trip.
Bring your paper prescription to the pharmacy or have your doctor send it directly to the UCHealth Longs Peak Hospital pharmacy.

UCHealth patients can request prescription refills through the My Health Connection website or through the UCHealth mobile app.

If an updated prescription is needed from your physician, our pharmacy will contact him or her.
Need to transfer a prescription to our pharmacy? Give us your current pharmacy information plus the list of medications you need, and we will take care of everything.
Our pharmacy participates with many different insurance companies. Because insurance plans, policies and laws often change, please contact your insurance company to see if we are a contracted provider.
Please notify us if you have an insurance plan change, coverage change or insurance loss, so we can properly bill your claims in a timely manner.
