The UCHealth Aspen Club, in partnership with Sharing the Care Campaign of Northern Colorado continue to highlight the importance of advanced health-care decision making (aka, discussing end-of-life issues). Advance Directives are legal documents that allow your doctors and loved ones to honor your wishes regarding your medical treatment. These documents only take effect when you are unable to make health care decisions for yourself.
The Aspen Club has many of the Advance Directive forms available and can help you through the process by notarizing the forms, helping you understand the form and also with many presentations including The Conversation Project, Get Your Advance Directives done, The Death Café and Go Wish Game and Discussion.
