Get proof of vaccination in the UCHealth app.
- Everyone vaccinated at UCHealth already has access to their information on the app.
- Sign in to your My Health Connection account.
- Tap “Your COVID-19 information” on the home screen after login.

Get proof of vaccination in the CLEAR app.
- Download the free CLEAR app and have a government-issued ID ready for when you’re prompted to create an account.
- Tap the blue “Digital vaccine card” button on the home screen.
- Choose “UCHealth Colorado” and follow instructions.
- Upload or scan your UCHealth SMART QR Code.
- Your verified digital vaccine card is ready for use.

Access your COVID-19 digital vaccine records and test results with ease.
A digital copy of your vaccine card and COVID-19 test results are available in the UCHealth app through your My Health Connection account so you don’t have to worry about carrying your paper card.
Plus, we’ve partnered with the CLEAR app so you can link your vaccine card for free, even if you’re not a CLEAR member. With both apps, you have digital access anywhere proof of vaccination is required, like airports, concerts, shops, restaurants and more.
Upload your CDC vaccination card into the CLEAR app.
If you were not vaccinated at UCHealth and don’t have a SMART Health Card QR Code, you can upload your CDC card to the CLEAR app by tapping the “CDC card” button and taking a photo.
