{"id":92586,"date":"2026-08-14T12:54:32","date_gmt":"2026-08-14T18:54:32","guid":{"rendered":"https:\/\/www.uchealth.org\/today\/?p=92586"},"modified":"2026-08-20T15:01:37","modified_gmt":"2026-08-20T21:01:37","slug":"ensuring-patients-are-fully-prepared-for-discharge-from-the-ed","status":"publish","type":"post","link":"https:\/\/www.uchealth.org\/today\/ensuring-patients-are-fully-prepared-for-discharge-from-the-ed\/","title":{"rendered":"Ensuring patients are fully-prepared for discharge from the ED"},"content":{"rendered":"<div style=\"margin-top: 0px; margin-bottom: 0px;\" class=\"sharethis-inline-share-buttons\" ><\/div><p>At the end of a patient\u2019s visit to an emergency department, there may be questions they hadn\u2019t previously felt comfortable asking, or they might not fully understand what they need to know for their care.<\/p>\n<p>In the five minutes taking their vital signs before discharge, the nurse can reconnect with the patient to address these needs, said Mandy Adkins, BSN, RN, CEN, TCRN, perioperative services nurse with <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-medical-center-of-the-rockies\/\">UCHealth Medical Center of the Rockies<\/a>.<\/p>\n<p>\u201cYou\u2019re one-on-one with the patient, and it slows you down enough to really address their concerns, recognize gaps in education and set them up for success,\u201d she said.<\/p>\n<p>In her previous role as a charge nurse with the <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-emergency-room-harmony\/\">UCHealth Emergency Room \u2013 Harmony Campus<\/a>, Adkins developed her UEXCEL Level III project to improve quality, safety and patient satisfaction in freestanding emergency departments (ED) in the UCHealth Northern Colorado Region.<\/p>\n<p>The project implemented a requirement for patients\u2019 vital signs to be entered into the electronic health record within 30 minutes of discharge, before the after-visit summary is printed. The change was rolled out across all UCHealth freestanding EDs.<\/p>\n<p>\u201cIn the emergency department, things move so quickly,\u201d Adkins said. \u201cIf you don\u2019t have vital signs before they leave, you can\u2019t prove that they\u2019re safe.\u201d<\/p>\n<p>The change led to a 22% increase in vital signs being recorded within the 30-minute window at the Harmony Campus location, and a 16% increase at the UCHealth Emergency Room \u2013 West Greeley, improving compliance with UCHealth\u2019s Standards of Nursing Practice.
