The line may be straight, with few obstacles: think the time-worn physician&#8217;s advice for enduring a common cold: \u201cRest and drink plenty of liquids.\u201d<\/p>\n<p>By contrast, the path for a person diagnosed with cancer may be rocky and challenge-strewn: side effects from chemotherapy, fatigue, worries about job loss and many others.<\/p>\n<p>Many patients with serious illnesses need more support from their medical providers than a straightforward explanation of their disease and treatments, said <a href=\"https:\/\/www.uchealth.org\/provider\/dan-matlock-md\/\">Dr. Daniel Matlock<\/a>, a professor of <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/geriatrics\" target=\"_blank\" rel=\"noopener\">Geriatric Medicine<\/a> at the University of Colorado School of Medicine.<\/p>\n<figure id=\"attachment_80730\" aria-describedby=\"caption-attachment-80730\" style=\"width: 200px\" class=\"wp-caption alignleft\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-80730\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2025\/02\/26134154\/patient-centered-decision-support-1-dan-matlock-web.webp\" alt=\"Dr. Dan Matlock has a longtime interest in involving patients in making decisions about their health. Photo by the University of Colorado.<\/figcaption><\/figure>\n<p>He acknowledged the expanding power of modern medicine to treat disease. But he noted an equally strong responsibility for providers: to listen to what patients and their loved ones want from their care. That means helping them make decisions that conform with their values, such as the kind of care and the quality of life they want.<\/p>\n<p>\u201cHow do we make sure,\u201d Matlock said, \u201cthat we are giving (treatments) to people who really want to go down that path with us and make sure they are fully informed?\u201d<\/p>\n<p>Matlock has pursued that question since he was a resident at the University of Colorado School of Medicine. He now directs the school\u2019s <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/accords\/cores-and-programs\/colorado-program-for-patient-centered-decisions\" target=\"_blank\" rel=\"noopener\">Colorado Program for Patient-Centered Decisions<\/a>, which is part of the Adult and Child Center for Outcomes Research and Delivery Science (<a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/accords\" target=\"_blank\" rel=\"noopener\">ACCORDS<\/a>). He and his program colleagues support patients and clinicians with initiatives like <a id=\"\" href=\"https:\/\/www.patientdecisionaid.org\/\" target=\"_blank\" rel=\"noopener\">iDecide<\/a>, which offers a host of free, universally available decision aids in areas spanning heart conditions, colon cancer screening, hospice and more.<\/p>\n<h2><strong>A broader plan to provide patient decision support<\/strong><\/h2>\n<figure id=\"attachment_80732\" aria-describedby=\"caption-attachment-80732\" style=\"width: 192px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-80732 size-full\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2025\/02\/26134206\/patient-centered-decision-support-2-russell-glasgow-web.webp\" alt=\"Dr. Russell Glasgow provides guidance in developing decision-support tools that will be implemented and sustained in clinical settings. Photo courtesy of University of Colorado.<\/figcaption><\/figure>\n<p>Now the ACCORDS faculty is about six months into a new series of projects to bolster patient decision-making power. The effort, funded by the <a id=\"\" href=\"https:\/\/cuanschutz.edu\/about\/leadership\/chancellor\/anschutz-acceleration-initiative\" target=\"_blank\" rel=\"noopener\">Anschutz Acceleration Initiative<\/a> (AAI), focuses on rapidly researching and creating tools to help patients make tough choices about their health, such as considering the risks and benefits of a procedure, managing multiple chronic diseases, understanding the benefits of their medications and other treatments and weighing where they receive their care.<\/p>\n<p>Matlock collaborated with <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/accords\/about\/faculty-staff-directory\/Glasgow-Russell-UCD6000016211\" target=\"_blank\" rel=\"noopener\">Dr. Russ Glasgow<\/a>, a PhD professor and researcher with the <a id=\"\" href=\"https:\/\/www.cumedicine.us\/?utm_source=google&amp;utm_medium=cpc&amp;utm_campaign=cu_med_paid&amp;gad_source=1&amp;gclid=Cj0KCQiA-5a9BhCBARIsACwMkJ7UXpAJGfrA6x6JHUjC7lV8ODiOEqfQ4l2LPuMGnOBhHSsJfVHJBd4aAkeQEALw_wcB\" target=\"_blank\" rel=\"noopener\">Department of Family Medicine<\/a>, in making the five-year funding proposal, which will eventually fund 10 projects. For now, the work focuses on crafting and evaluating decision-support tools in four areas:<\/p>\n<ul>\n<li>Heart transplant<\/li>\n<li>Alzheimer\u2019s disease and related dementias<\/li>\n<li>Carotid artery stenting<\/li>\n<li>Primary care patients with chronic conditions<\/li>\n<\/ul>\n<h2><strong>An emphasis on rapid development and evaluation of support tools<\/strong><\/h2>\n<p>The ACCORDS <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/accords\/cores-and-programs\/dissemination-implementation-science-program\" target=\"_blank\" rel=\"noopener\">Dissemination &amp; Implementation Science Program<\/a>, which Glasgow heads, is a key element of all four projects. Glasgow\u2019s team works with members of each project to rapidly design, review and adapt accessible materials \u2013 booklets, web applications, and questionnaires, for example \u2013 that clinics, hospitals, medical offices, and others can use both widely and consistently. Patients, clinicians and community members all make contributions.<\/p>\n<p>\u201cSo many interventions are designed and work in the laboratory or very tightly controlled conditions,\u201d Glasgow said. Moved to the everyday world, however, these programs often collect dust, he added. Perhaps they don\u2019t appeal to diverse patient populations, for example. Others may be impractical \u201cto sustain in the face of competing demands and changing clinical priorities,\u201d he said.<\/p>\n<p>The finances of expanding improvement projects present another big barrier, Glasgow added.<\/p>\n<p>\u201cThere has been far too little consideration of the cost (of tools and the delivery), from the planning stage to implementation,\u201d he said. For the AAI project, <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/accords\/about\/faculty-staff-directory\/Creel-Liza-UCD0438\" target=\"_blank\" rel=\"noopener\">Dr. Liza Creel<\/a>, with the ACCORDS <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/health-care-policy-and-research\" target=\"_blank\" rel=\"noopener\">Division of Health Care Policy and Research<\/a>, is leading work to manage the economic cost of the decision-support tools that are being developed for implementation.<\/p>\n<p>Patient advisory groups provide input for each of the projects, and a Spanish-speaking patient panel also addresses specific concerns of that community. <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/accords\/about\/faculty-staff-directory\/Perez-Jolles-Monica-UCD6551\" target=\"_blank\" rel=\"noopener\">Dr. Monica P\u00e9rez Jolles<\/a> of ACCORDS and associate professor in the <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/pediatrics\" target=\"_blank\" rel=\"noopener\">Department of Pediatrics<\/a> with the University of Colorado School of Medicine, leads that effort.<\/p>\n<p>The goals for each project are the same: help patients and families make informed decisions aligned with their values and situations, Matlock said. However, their strategies differ.<\/p>\n<h2><strong>Decision aids for patients considering heart transplantation<\/strong><\/h2>\n<figure id=\"attachment_80731\" aria-describedby=\"caption-attachment-80731\" style=\"width: 200px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-80731\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2025\/02\/26134159\/patient-centered-decision-support-3-colleen-mcilvennan-web.webp\" alt=\"Dr. Colleen McIlvennan is a member of the team working on a decision aid for patients considering heart transplant. It is\u00a0 led by <a id=\"\" href=\"https:\/\/som.cuanschutz.edu\/Profiles\/Faculty\/Profile\/13824\" target=\"_blank\" rel=\"noopener\">Dr. Larry Allen<\/a>, professor in the <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/cardiology\" target=\"_blank\" rel=\"noopener\">Division of Cardiology<\/a>, and medical director of the <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/cardiology\/clinical-programs\/Advanced-Heart-Failure-MCS-LVAD-Transplant\" target=\"_blank\" rel=\"noopener\">Advanced Heart Failure program<\/a> at the University of Colorado, with support from division colleagues <a id=\"\" href=\"https:\/\/som.cuanschutz.edu\/Profiles\/Faculty\/Profile\/7654\" target=\"_blank\" rel=\"noopener\">Dr. Colleen McIlvennan<\/a> and <a id=\"\" href=\"https:\/\/som.cuanschutz.edu\/Profiles\/Faculty\/Profile\/21114\" target=\"_blank\" rel=\"noopener\">Laura Peters<\/a>, and ACCORDS research services professional <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/accords\/about\/staff-directory\/Viny-Mikayla-UCD6004061679\" target=\"_blank\" rel=\"noopener\">Mikayla Viny<\/a>.<\/p>\n<p>The effort builds on previous work by Allen, McIlvennan and colleagues that produced an <a id=\"\" href=\"https:\/\/www.patientdecisionaid.org\/lvad\/\" target=\"_blank\" rel=\"noopener\">informational pamphlet and video<\/a> to assist heart failure patients considering left-ventricular assist device (LVAD) implantation.<\/p>\n<p>A national trial of the LVAD decision aids showed that they \u201cimproved patient knowledge\u201d and increased \u201cconcordance\u201d between their stated values and the care they ultimately received, McIlvennan said.<\/p>\n<p>\u201cI think big picture, we had success in having (the LVAD aid) focused on both patients and caregivers, and so we have modeled that in the transplant decision aid,\u201d she added.<\/p>\n<p>The days when clinicians gave patients terse and hard to understand instructions about their care and moved on are rapidly fading, particularly with life-and-death procedures like heart transplantation, Peters said.<\/p>\n<p>\u201cHealth care has shifted toward patient engagement and autonomy,\u201d she said. However, it can be difficult for patients to find accurate and understandable information to guide decisions about their care, she added.<\/p>\n<p>\u201cEven the most conscientious provider may find it challenging to deliver complex guidance during a short visit,\u201d Peters said. \u201cAn aid that summarizes accurate scientific information and the pros and cons of different decisions help to provide a cohesive understanding of what transplant is.\u201d<\/p>\n<p>The aid had been through nine iterations as of mid-January, with input from patients and clinicians, Viny said. It aims to explain the long-term risks and benefits of transplantation \u2013 not all patients will choose to get one \u2013 and also incorporate questions from patient advisors about how to maintain their mental health, she said.<\/p>\n<p>\u201cWe\u2019ve expanded on that section in the decision aid,\u201d Viny said.<\/p>\n<h2><strong>Road-testing a &#8216;roadmap&#8217; for dementia care<\/strong><\/h2>\n<p>Matlock leads this project, along with <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/accords\/about\/faculty-staff-directory\/Scherer-Laura-UCD6002372739\" target=\"_blank\" rel=\"noopener\">Dr. Laura Scherer<\/a>, associate professor in the Division of Cardiology, and <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/accords\/about\/faculty-staff-directory\/Tate-Channing-UCD23231\" target=\"_blank\" rel=\"noopener\">Dr. Channing Tate<\/a>, assistant professor in the <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/general-internal-medicine\" target=\"_blank\" rel=\"noopener\">Division of Internal Medicine<\/a>. It is modeled on a <a id=\"\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/38911020\/\" target=\"_blank\" rel=\"noopener\">\u201croadmap\u201d for coronary artery disease patients<\/a> developed by Scherer and two colleagues to address patients\u2019 misconceptions about the disease and encourage healthy behaviors.<\/p>\n<p>Matlock and his team are following that lead with a roadmap to help patients with dementia and their caregivers anticipate and address \u201cdomains of care,\u201d such as types of dementia, how it progresses, home safety, medication storage, self-care and more. Representatives from the Alzheimer\u2019s Association have provided input, along with a patient panel and clinicians.<\/p>\n<p>\u201cWe\u2019re trying to make sure we use gentle language that talks about some of the tough things that could happen in the future in a very patient-centered and acceptable way,\u201d Matlock said.<\/p>\n<h2><strong>Carotid artery stenting: Meeting a (possible) demand for shared decision-making<\/strong><\/h2>\n<p><a id=\"\" href=\"https:\/\/som.cuanschutz.edu\/Profiles\/Faculty\/Profile\/27631\" target=\"_blank\" rel=\"noopener\">Dr. Christopher Knoepke<\/a>, assistant research professor in the\u00a0Division of Cardiology,\u00a0leads the project with\u00a0<a href=\"https:\/\/www.uchealth.org\/provider\/michelle-fullard-md-msce\/\">Dr. Michelle Fullard<\/a>, assistant professor in the\u00a0<a href=\"https:\/\/medschool.cuanschutz.edu\/neurology\">Department of Neurology<\/a>. It addresses a <a id=\"\" href=\"https:\/\/www.cms.gov\/medicare-coverage-database\/view\/ncacal-decision-memo.aspx?proposed=N&amp;NCAId=311\" target=\"_blank\" rel=\"noopener\">proposed mandate<\/a> by the Centers for Medicare and Medicaid Services.<\/p>\n<p>The mandate would require that providers \u201cengage in a formal shared decision-making interaction\u201d with patients before they undergo surgery to open narrowed carotid arteries with stents. The discussions must include risks and benefits of the stenting procedures and other options, including <a id=\"\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK470582\/\" target=\"_blank\" rel=\"noopener\">carotid endarterectomy<\/a> to surgically remove plaque from the artery, and medical therapy.<\/p>\n<figure id=\"attachment_80733\" aria-describedby=\"caption-attachment-80733\" style=\"width: 200px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-80733\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2025\/02\/26134208\/patient-centered-decision-support-4-kelsey-eklund-web.webp\" alt=\"Dr. Kelsey Eklund and her colleagues are designing a support tool for patients with symptoms of carotid artery disease. Photo courtesy of University of Colorado.<\/figcaption><\/figure>\n<p>The ACCORDS team is preparing for the proposed mandate by developing and testing a decision-making aid for patients with <a id=\"\" href=\"https:\/\/www.cumedicine.us\/health-insights\/carotid-artery-disease-risk-factors-prevention-and-treatment\" target=\"_blank\" rel=\"noopener\">symptomatic carotid artery disease<\/a>, said <a href=\"https:\/\/www.uchealth.org\/provider\/kelsey-eklund-md\/\">Dr. Kelsey Eklund<\/a>, assistant professor in the Department of Neurology. That\u2019s vital because these patients and their caregivers must absorb detailed information about multiple treatment options, sometimes after a stroke or another life-threatening event.<\/p>\n<p>Eklund said the decision aid, now being developed and revised, encourages patients to ask their clinicians important questions, such as why they are recommending a procedure, what they can expect from their recovery, and how to reduce their long-term risk of stroke and other problems.<\/p>\n<p>\u201cFor me and our group, (the work) is about making our patients feel better about their decisions, and feel like they fully understand what they are getting into or what they are saying \u2018no\u2019 to,\u201d she concluded. \u201cWe also have an opportunity to highlight that while it is our recommendation that you have a procedure, you can ultimately decide not to have it if it is not aligned with your goals, values and situation.\u201d<\/p>\n<p>In some patients, carotid artery disease can be <a id=\"\" href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2775719#:~:text=Asymptomatic%20carotid%20artery%20stenosis%20refers,population%20but%20increases%20with%20age.\" target=\"_blank\" rel=\"noopener\">asymptomatic<\/a>. That requires a separate decision aid, Eklund said. The ACCORDS team is collaborating with Dartmouth Health, which is working on a guide for those patients. Dartmouth Health has done <a id=\"\" href=\"https:\/\/vascularspecialistonline.com\/carotid-disease-getting-to-the-right-decision\/\" target=\"_blank\" rel=\"noopener\">significant research and work<\/a> on creating an aid that can be used by different specialists \u2013 cardiologists, neurologists, neurosurgeons and interventional radiologists \u2013 to discuss carotid artery disease with patients.<\/p>\n<h2><strong>Helping primary care patients prioritize and improve their health risks <\/strong><\/h2>\n<figure id=\"attachment_80729\" aria-describedby=\"caption-attachment-80729\" style=\"width: 200px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-80729\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2025\/02\/26134149\/patient-centered-decision-support-5-amy-huebschmann-web.webp\" alt=\"Dr. Amy Huebschmann and Glasgow lead a team developing tools to help patients with chronic conditions identify the most important goals for their care. Photo courtesy of University of Colorado.<\/figcaption><\/figure>\n<p><a href=\"https:\/\/www.uchealth.org\/provider\/amy-huebschmann-md\/\">Dr. Amy Huebschmann<\/a>, a primary care physician and professor in the Division of Internal Medicine, leads this project along with Dr. Glasgow. Huebschmann said researchers at the National Academy of Medicine have for the past few years been \u201cdigging into to addressing what matters most to patients across several dimensions of health risks.\u201d\u00a0 That\u2019s a tough task in a relatively short clinic visit that leaves little time for patients to ask questions, get feedback on progress they are making with their health and set top goals for improvement, she said.<\/p>\n<p>The dilemma deepens for clinicians treating patients with chronic conditions like diabetes. The most effective approach, Huebschmann said, is to address \u201c<a id=\"\" href=\"https:\/\/www.nationalacademies.org\/our-work\/transforming-health-care-to-create-whole-health-strategies-to-assess-scale-and-spread-the-whole-person-approach-to-health\" target=\"_blank\" rel=\"noopener\">the whole health of the patient<\/a>.\u201d That includes not only their symptoms, but also lifestyle behaviors, like smoking, and \u201cunmet social needs,\u201d such as getting healthy food and reliable transportation.<\/p>\n<p>But how to do that in a time-restricted environment when patients typically have several risk factors, and may also be managing multiple chronic conditions?<\/p>\n<p>\u201cWe\u2019ve been doing workshops with clinicians and patient advisory groups to talk through questions that are part of these \u2018whole health\u2019 attributes,\u201d with the UCHealth Patient Experience team also providing input, Huebschmann said. The goal: identify the risk factors that are most important to address and how many questions it is feasible to include.<\/p>\n<p>The work led to developing a web application for patients to complete <em>before<\/em> a clinic visit. Patients identify their most important health concerns and convey them to their primary care team, increasing the amount of productive time available.<\/p>\n<p>\u201cGetting this information into (patients\u2019) charts will allow clinicians and patients to review together what matters most,\u201d Huebschmann said.<\/p>\n<p>This web application will be modeled on the earlier <a href=\"https:\/\/www.uchealth.org\/provider\/amy-huebschmann-md\/\">MyOwnHealthReport<\/a> website, which has also been developed and tested to address whole-person care by Huebschmann and other collaborators.<\/p>\n<p>\u201cPast trials have shown that earlier versions of <a href=\"https:\/\/www.uchealth.org\/provider\/amy-huebschmann-md\/\">MyOwnHealthReport<\/a> helped patients set goals, increase their activation and interest to work on them, as well as make short-term changes in health behaviors,\u201d Huebschmann said.<\/p>\n<p>As for primary care clinicians, Huebschmann said that her team has heard \u201cloud and clear\u201d that they want the information that patients submit to be easily accessible within the UCHealth Epic electronic health record (EHR). To that end, the team is working with the UCHealth Patient Experience team and their information technology specialists to meet that goal of creating a \u201cfrictionless interface\u201d that allows providers and patients to make the most of their time together, she said.<\/p>\n<p>\u201cWe really want to address what matters most to patients and clinicians,\u201d Huebschmann said. \u201cWe know it\u2019s not going to be the same for every patient.
