Photo: UCHealth.<\/figcaption><\/figure>\n<p>Establishing a relationship with a primary care provider (also called a PCP) means so much more than merely getting an appointment when you\u2019re not feeling well.<\/p>\n<p>According to medical experts, a sustained and trusting partnership between a patient and a primary care provider results in better health care outcomes for individuals and their communities.<\/p>\n<p>\u201cGood primary care providers support patients with complex conditions, urgent care issues and everything in between. It\u2019s not just sprained ankles and colds,\u201d said <a href=\"https:\/\/www.uchealth.org\/provider\/brian-harrington-md\/\">Dr. Brian Harrington<\/a>, <span data-teams=\"true\">a family medicine physician with <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-primary-care-steamboat-springs\/\">UCHealth Primary Care &#8211; Steamboat Springs<\/a><\/span>.<\/p>\n<p>The primary care specialties include family medicine, general internal medicine and pediatrics.<\/p>\n<h2 class=\"wp-block-heading\"><strong>What does a primary care provider do?<\/strong><\/h2>\n<p>According to Harrington, primary care uniquely combines a number of aspects, including disease prevention and screening; wellness and preventive care; chronic disease management; coordination of care among health care providers; and integrated care. Additionally, primary care providers work to develop a sustained health partnership over time with patients. They place on emphasis on community and public health.<\/p>\n<h2 class=\"wp-block-heading\"><strong>How can having a primary care provider improve your health?<\/strong><\/h2>\n<p>\u201cA strong PCP relationship equals stronger health care combined with lower health care costs and better outcomes,\u201d Harrington said.<div class=\"su-pullquote su-pullquote-align-right\"><a href=\"https:\/\/www.uchealth.org\/today\/how-to-find-a-primary-care-doctor\/\">Tips for finding a primary care provider<\/a> you can trust. <\/div>\n<p>He emphasizes the importance of establishing a primary care home as soon as possible, laying the groundwork for a collaborative relationship by communicating with your PCP early and often, and sharing health information. This allows them to review your history over time, notice changes or patterns in your health, and identify new symptoms.<\/p>\n<p>\u201cEverybody should establish a primary care medical home,\u201d said Harrington. \u201cThat\u2019s where you will have those important medical conversations, and a personalized patient-centered plan can be created for you. It\u2019s not about a specific organ or part of your body, but about the whole person.\u201d<\/p>\n<h2 class=\"wp-block-heading\"><strong>The importance of having a medical home<\/strong><\/h2>\n<p>When you establish a medical home with a primary care provider, they become your location for:<div class=\"su-pullquote su-pullquote-align-right\">Females: Do you need a OB-GYN and a PCP? <a href=\"https:\/\/www.uchealth.org\/today\/do-i-need-an-ob-gyn-and-a-pcp\/\">Learn more.<\/a> <\/div>\n<ul>\n<li>Physicals and health screenings on an annual basis, or whatever specific and consistent health maintenance plan your PCP determines is best. \u201cYou don\u2019t want to have to find a place just when you have crises,\u201d said Harrington.<\/li>\n<li>A repository for your medical records. \u201cThis is the place where your care begins,\u201d he said, \u201cwhere your care exists, where your records are kept with someone who knows you.\u201d<\/li>\n<li>Management of chronic diseases and conditions, such as high blood pressure, diabetes, asthma, obesity and tobacco use. \u201cAbout 80% of health outcomes are determined by the time you are 60 \u2026 when you are in your 20s and 30s,\u201d said Harrington. \u201cWe need to have those conversations with patients so they know how important those lifestyle decisions are, so they can maintain a high quality of life as they age.\u201d<\/li>\n<li>An <a href=\"https:\/\/www.uchealth.org\/today\/behavioral-health-in-primary-care-clinics-now-available-to-make-it-easier-to-get-help-for-anxiety-depression-and-other-behavioral-health-challenges\/\">integrated health care model<\/a> where a patient can also find help such as physical and occupational therapies, mental health services, a pharmacy, and care coordinators and referral managers who often act as patient advocates. \u201cWe have evolved beyond just a doctor and a patient,\u201d he said, \u201cand we meet people where they\u2019re at.\u201d<\/li>\n<li>And perhaps most importantly, where you will receive wellness care, not just disease care. \u201cWe want patients to maintain their health,\u201d said Harrington. \u201cOur goal is to practice disease detection early, before it begins to impact the life of the patient.\u201d<\/li>\n<\/ul>\n<h2 class=\"wp-block-heading\"><strong>Emphasis on community<\/strong><\/h2>\n<p>After practicing medicine for three decades, Harrington said it\u2019s rewarding to see patients at different stages of their lives and recall health issues from previous years.<\/p>\n<p>\u201cThat type of sustained continuity of care over the long term is an important concept of being a PCP,\u201d he said. \u201cWe can be focused on the value of care we provide and not just the volume of care.\u201d<\/p>\n<p>Primary care providers have a broad skill set and are involved in, and live in, the places where they practice, he said.<\/p>\n<p>\u201cWe care about community health and how we should tend to that community, whether it\u2019s through equity of care and access to care, food equity, and issues such as wellness, physical activity and recreation opportunities,\u201d said Harrington. \u201cWhen patients need health care, a primary care home is the one place where they can meet the majority of their needs.\u201d<\/p>\n<p><i>This story first appeared in the Steamboat Pilot.<\/i><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Establishing a relationship with a primary care provider (also called a PCP) means so much more than merely getting an appointment when you\u2019re not feeling well.
