{"id":71346,"date":"2023-08-16T09:38:59","date_gmt":"2023-08-16T15:38:59","guid":{"rendered":"https:\/\/www.uchealth.org\/today\/?p=71346"},"modified":"2023-08-22T08:31:27","modified_gmt":"2023-08-22T14:31:27","slug":"gi-geniuss-ai-assists-in-colonoscopies","status":"publish","type":"post","link":"https:\/\/www.uchealth.org\/today\/gi-geniuss-ai-assists-in-colonoscopies\/","title":{"rendered":"GI Genius\u2019s AI helps experts spot problems during colonoscopies"},"content":{"rendered":"<div style=\"margin-top: 0px; margin-bottom: 0px;\" class=\"sharethis-inline-share-buttons\" ><\/div><p>Artificial intelligence (AI) has arrived in the colonoscopy procedure rooms of <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-university-of-colorado-hospital-uch\/\">UCHealth University of Colorado Hospital<\/a> (UCH) on the <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-at-university-of-colorado-anschutz-medical-campus\/\">Anschutz Medical Campus<\/a>.<\/p>\n<p>It\u2019s not taking over, but rather assisting gastroenterologists in spotting polyps that can become cancerous.<\/p>\n<p>The AI comes inside Medtronic\u2019s <a id=\"\" href=\"https:\/\/www.medtronic.com\/covidien\/en-us\/products\/gastrointestinal-artificial-intelligence\/gi-genius-intelligent-endoscopy.html\" target=\"_blank\" rel=\"noopener\">GI Genius<\/a> intelligent endoscopy module. It\u2019s an unassuming blue box that hooks into standard endoscopy tools in three of the UCH\u2019s five colonoscopy procedure rooms, but it packs some serious diagnostic power.<\/p>\n<figure id=\"attachment_71348\" aria-describedby=\"caption-attachment-71348\" style=\"width: 400px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-71348\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2023\/08\/16092610\/Medtronic-GI-Genius-Docseee.webp\" alt=\"AI assisted colonoscopies are happening at UCHealth and helping doctors identify even small polyps.\" width=\"400\" height=\"282\" \/><figcaption id=\"caption-attachment-71348\" class=\"wp-caption-text\">Medtronic\u2019s GI Genius, now available at UCHealth University of Colorado Hospital, uses AI to help gastroenterologists spot precancerous polyp detection during colonoscopies. Image courtesy of Medtronic.<\/figcaption><\/figure>\n<p>\u201cI don\u2019t see how this isn\u2019t standard of care at some relatively short interval,\u201d said <a href=\"https:\/\/www.uchealth.org\/provider\/blair-fennimore-md-gastroenterology\/\">Dr. Blair Fennimore<\/a>, a University of Colorado School of Medicine gastroenterologist.<\/p>\n<p>The U.S. Food and Drug Administration (FDA) <a id=\"\" href=\"https:\/\/www.fda.gov\/news-events\/press-announcements\/fda-authorizes-marketing-first-device-uses-artificial-intelligence-help-detect-potential-signs-colon\" target=\"_blank\" rel=\"noopener\">approved<\/a> GI Genius in April 2021 based on a 2020 <a id=\"\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32371116\/\" target=\"_blank\" rel=\"noopener\">study<\/a> that demonstrated that it can spot small, potentially precancerous lesions called adenomas \u2013 colorectal polyps \u2013 that gastroenterologists missed. A 2022 study <a id=\"\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35304117\/\" target=\"_blank\" rel=\"noopener\">showed<\/a> even stronger results: GI Genius reduced the number of missed polyps by about a factor of two.<\/p>\n<p>Those numbers underscore how hard it is even for well-trained eyes to spot tiny polyps. They are often scarcely differentiated in color or texture from the colon\u2019s mucosal layers, and they\u2019re often less than 0.5 millimeters (0.2 inches) across, or about the height of a computer USB connector.<\/p>\n<h2><strong>AI-assisted colonoscopies<\/strong><\/h2>\n<p>GI Genius system works by tapping into the endoscope\u2019s feed before it hits the screen the physician observes during the colonoscopy. If GI Genius spots a suspected polyp, it puts a digital green box around it onscreen. The gastroenterologist then makes the call as to whether it\u2019s actually a polyp and, if so, whether to remove it during the procedure.<\/p>\n<p>The studies showed GI Genius to have little advantage over trained physicians regarding larger, more dangerous polyps. But small polyps can grow into problem ones: For each 1% increase in detecting these adenomas, the risk of being diagnosed with colon cancer in the next five years <a id=\"\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/24693890\/\" target=\"_blank\" rel=\"noopener\">falls<\/a> by 3%.<\/p>\n<p>More than 150,000 colorectal cancer cases are diagnosed annually in the United States alone, making it the third-most common form of cancer. The five-year <a id=\"\" href=\"https:\/\/www.cancer.org\/cancer\/types\/colon-rectal-cancer\/detection-diagnosis-staging\/survival-rates.html\" target=\"_blank\" rel=\"noopener\">survival rate<\/a> for localized colon cancer detected early is 91%. That drops to 72% if it\u2019s spread to nearby structures or lymph nodes and plummets to 13% if it ends up in distant organs such as the liver or lungs. Finding more polyps, earlier, can make a real difference.<\/p>\n<figure id=\"attachment_71349\" aria-describedby=\"caption-attachment-71349\" style=\"width: 200px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-71349\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2023\/08\/16092733\/Dr-Blair-Fennimore.jpgeee.webp\" alt=\"Dr. Blair Fennimore\" width=\"200\" height=\"250\" \/><figcaption id=\"caption-attachment-71349\" class=\"wp-caption-text\">Dr. Blair Fennimore<\/figcaption><\/figure>\n<p>Fennimore says that 30% to 50% of screening colonoscopies spot an adenoma. He says it\u2019s hard to know whether GI Genius has picked up adenomas that he would have missed, mainly because the green boxes appear so quickly on the screen. But, he says, \u201cI\u2019m convinced that there were small polyps that I would have missed \u2013 ones that blended in and looked exactly like the background mucosa.\u201d<\/p>\n<p>That there have also been instances where he noted a polyp that GI Genius missed, he says, and times when GI Genius puts a green box around something he decides isn\u2019t a concern.<\/p>\n<p>\u201cIt\u2019s not the end-all, be-all,\u201d Fennimore said. \u201cBut it\u2019s a tool to decrease your rates of missed polyps.\u201d<\/p>\n<p>He expects that tool to continue to improve: Because GI Genius is AI-driven, it will improve with increasing volumes of real-world data to train on. He also anticipates that UCHealth will acquire more of the devices at UCH and elsewhere in the system.<\/p>\n<p>Do gastroenterologists feel threatened by that prospect?<\/p>\n<p>\u201cAs a GI doctor and somebody who&#8217;s done thousands of colonoscopies, it&#8217;s almost like, \u2018I don&#8217;t want there to be a computer better than me. I feel like I do a good job of this,\u201d Fennimore said. \u201cBut this is going to make me better. We&#8217;re in the business of preventing colon cancer, not of massaging egos.
