Photo: Getty Images<\/figcaption><\/figure>\n<p>For some 20 years, <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/center-for-womens-health-research\/about-us\/-researchers\/kerrie-moreau\" target=\"_blank\" rel=\"noopener\">Professor Kerrie Moreau, <\/a>Ph.D., studied the physiology of cardiovascular aging: the processes that stealthily prod the blood vessels to stiffen and clog with plaque and the heart\u2019s pumping power to diminish. For example, she\u2019s probed the roles of sex hormones in that process, with an eye toward determining how they might be modified to prolong life.<\/p>\n<p>In recent years, Moreau, professor of <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/geriatrics\" target=\"_blank\" rel=\"noopener\">Geriatric Medicine<\/a> at the University of Colorado School of Medicine, has also trained her attention on how psychological stress and trauma might also accelerate cardiovascular decline.<\/p>\n<p>\u201cWe study the underlying biology that increases the susceptibility for blood vessel health to deteriorate,\u201d Moreau says. \u201cBut how do life stressors contribute to that?\u201d<\/p>\n<figure id=\"attachment_67963\" aria-describedby=\"caption-attachment-67963\" style=\"width: 200px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-67963 size-full\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2023\/01\/24134136\/NightWare-Trial-1-Kerrie-Moreaueee.webp\" alt=\"A photo of Kerrie Moreau, who is helping to conduct a study on NightWare.\" width=\"200\" height=\"300\" \/><figcaption id=\"caption-attachment-67963\" class=\"wp-caption-text\">Kerrie Moreau, PhD<\/figcaption><\/figure>\n<p>Working as a research health scientist at the <a id=\"\" href=\"https:\/\/www.va.gov\/eastern-colorado-health-care\/\" target=\"_blank\" rel=\"noopener\">VA Eastern Colorado Health Care System<\/a>, Moreau has seen many men and women struggling with the effects of <a href=\"https:\/\/www.uchealth.org\/diseases-conditions\/adjustment-disorders\/\">post-traumatic stress disorder<\/a> (PTSD): a reaction to a disturbing event that brings lingering psychological challenges well after a person has experienced it. She is interested in exploring the possible physical consequences of PTSD. It\u2019s far from idle speculation. The Veterans Administration, for example, is keenly interested in the question, noting that <a id=\"\" href=\"https:\/\/www.ptsd.va.gov\/publications\/rq_docs\/V28N1.pdf\" target=\"_blank\" rel=\"noopener\">studies have linked PTSD<\/a> to an increased risk of developing cardiovascular disease.<\/p>\n<p>Moreau is now delving into the possible connection. She leads a <a id=\"\" href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT05365607\" target=\"_blank\" rel=\"noopener\">National Institute on Aging-funded study<\/a> of a <a id=\"\" href=\"https:\/\/aasm.org\/fda-approves-nightware-app-treat-nightmares\/\" target=\"_blank\" rel=\"noopener\">device approved in 2020 by the FDA<\/a> to help treat people with PTSD who suffer from sleep disturbances caused by nightmares. Moreau\u2019s study aims to determine if <a id=\"\" href=\"https:\/\/nightware.com\/about\/\" target=\"_blank\" rel=\"noopener\">the device<\/a>, called NightWare, can also help to reduce stiffness in arteries and improve blood pressure, heart rate and other nervous system functions.<\/p>\n<h2><strong>NightWear collects data in an effort to improve health<\/strong><\/h2>\n<p>NightWare is a \u201c<a id=\"\" href=\"https:\/\/www.premera.com\/medicalpolicies\/13.01.500.pdf\" target=\"_blank\" rel=\"noopener\">prescription digital therapeutic<\/a>,\u201d meaning it uses mobile technology, such as a phone or tablet, and must be prescribed by a health care provider (no free downloads). Individuals are prescribed an iPhone and an Apple Watch equipped to monitor and collect information about body movement and heart rate while they sleep.<\/p>\n<p>The idea is that the data, gathered over time, helps the system to establish a NightWare user\u2019s sleep patterns and build a profile of a typical slumber. When the system detects disruptions to the pattern that could be signs of a nightmare \u2013 tossing and turning or rapidly quickening heart rate, for example \u2013 the watch vibrates, jogging the individual from the sleep turbulence without waking them.<\/p>\n<figure id=\"attachment_67964\" aria-describedby=\"caption-attachment-67964\" style=\"width: 250px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-67964\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2023\/01\/24134218\/NightWare-Trial-2-Ryan-Hollidayeee.webp\" alt=\"A photo of Ryan Holliday, who is helping to conduct a study on NightWare.\" width=\"250\" height=\"358\" \/><figcaption id=\"caption-attachment-67964\" class=\"wp-caption-text\">Ryan Holliday, PhD<\/figcaption><\/figure>\n<p>The potential benefits of simply sleeping better for a person with PTSD are enormous, Moreau said. Good sleep restores both mind and body, which improves quality of life. A rested person is more likely to exercise, she added. \u201cCollectively, better sleep may help with depression, substance use disorders and cardiovascular health.\u201d<\/p>\n<p>Whether sleep improvements drive positive changes in the body\u2019s processes is the fundamental question of Moreau\u2019s randomized study, which is now underway. It aims to enroll 50 people, half of whom will receive the NightWare device, the other half a \u201csham\u201d that is not enabled.<\/p>\n<p>Participants must be diagnosed with PTSD, using criteria from the <a id=\"\" href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT05365607\" target=\"_blank\" rel=\"noopener\">fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5)<\/a>. In addition, they will complete at-home sleep studies, both before and after the trial, to gauge changes in sleep patterns.<\/p>\n<p>The study will glean data about possible physiologic and biologic changes among participants, as well as information about quality of life, suicide risk and depression symptoms.<\/p>\n<h2><strong>Addressing an epidemic of stress and trauma<\/strong><\/h2>\n<p>Moreau said that as soon as she learned of the FDA\u2019s approval of NightWare in November 2020, she reached out to the company, which responded favorably to exploring whether it could be used to help people with PTSD decrease their cardiovascular risk.<\/p>\n<p>She added that she immediately saw that the usefulness of the device could extend well beyond the veterans&#8217; community. The COVID-19 pandemic was creating enormous stress on front-line health care workers, as well as friends and families of patients. Other societal pressures have continued to mount, she added.<\/p>\n<p>\u201cWe have COVID-19, natural disasters, mass shootings, sexual and physical abuse, and political unrest,\u201d Moreau said. \u201cMost people think of PTSD as affecting combat veterans, but it affects so many more people than that.\u201d<\/p>\n<p>Trauma and PTSD have a variety of causes, agreed <a id=\"\" href=\"https:\/\/som.cuanschutz.edu\/Profiles\/Faculty\/Profile\/27823\" target=\"_blank\" rel=\"noopener\">Ryan Holliday<\/a>, Ph.D., assistant professor in the <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/psychiatry\" target=\"_blank\" rel=\"noopener\">Department of Psychiatry at CU<\/a> and co-investigator on Moreau\u2019s study. He noted people can experience sleep disruptions, including nightmares and prolonged insomnia, following trauma without necessarily having PTSD. However, he added, \u201cIn my clinical and research experience, sleep disruptions are almost always associated with PTSD.\u201d<\/p>\n<p>The consequences of those disruptions can be profound, Holliday added. For example, a person suffering from PTSD-related nightmares who is having trouble falling asleep may turn to alcohol for help.<\/p>\n<p>\u201cThey may get into a pattern of thinking, \u2018I can\u2019t fall asleep unless I drink,\u2019\u201d Holliday said. \u201cThen they have to drink more and more to fall asleep.\u201d<\/p>\n<h2><strong>Attempt to supplement existing care<\/strong><\/h2>\n<p>The FDA stressed that NightWare is not a stand-alone therapy for PTSD. In fact, Holliday said, providers have well-established treatments for breaking sleepless cycles for people burdened by PTSD. These include anti-depressant and sleep medications and talk therapies, such as <a id=\"\" href=\"https:\/\/www.apa.org\/ptsd-guideline\/treatments\/prolonged-exposure\" target=\"_blank\" rel=\"noopener\">prolonged exposure<\/a> and <a id=\"\" href=\"https:\/\/www.apa.org\/ptsd-guideline\/treatments\/cognitive-processing-therapy\" target=\"_blank\" rel=\"noopener\">cognitive processing<\/a>. Cognitive behavioral therapy can also help treat insomnia.<\/p>\n<p>\u201cI\u2019m a big believer in these treatments,\u201d Holliday said. \u201cWe know they work better than other treatments, and they help people get better relatively quickly.\u201d<\/p>\n<p>He added, however, that the therapies don\u2019t work for some people, while others may not stick with them. The challenge is to also find \u201caugments\u201d to treatment that \u201cwill help to increase the number of people who benefit.\u201d<\/p>\n<p>NightWare may or may not be one of those PTSD treatment augments, Holliday said, but data about its effectiveness were \u201cinitially promising.\u201d He also said the device benefits from being \u201crelatively easy\u201d to implement, with little disruption to patients\u2019 lives. That increases the chances that they will use it regularly.<\/p>\n<p>\u201cIf we can develop and design small things we can add to treatment, that could give us a bigger return on our investment,\u201d Holliday said. Having more options, in turn, could help people prevent their PTSD from progressing before they seek help, he said.<\/p>\n<p>\u201cWe want to catch it before symptoms become rampant and impact people in so many different domains,\u201d Holliday said.<\/p>\n<p>Could the cardiovascular and nervous systems be among those damaged domains? Moreau hopes to find out. She added that she plans to continue studying the \u201cbiologic pathways\u201d that underlie cardiovascular disease and searching for clues that help the development of supplements, medications and other therapies.
