Photo: Getty Images.<\/figcaption><\/figure>\n<p>The COVID-19 pandemic has taken the world on a wild ride.<\/p>\n<p>There have been surges, plateaus, ebbs and flows of infections, illnesses, hospitalizations and deaths as the SARS-CoV-2 virus cuts its unpredictable path. For now, at least, the wave of cases is in sharp decline.<\/p>\n<p>Yet even if the virus were to somehow vanish tomorrow, legions of individuals who survived COVID-19 would be left with mysterious reminders of their infections, collectively known as long COVID.<\/p>\n<p><a id=\"\" href=\"https:\/\/www.cdc.gov\/covid\/long-term-effects\/\" target=\"_blank\" rel=\"noopener\">According to the Centers for Disease Control and Prevention<\/a>, these COVID \u201clong haulers\u201d are those who experience symptoms four weeks or longer after the original infection. After that, much remains murky about long COVID. Those who spent long stints in intensive care units, many on ventilators, are at obvious risk of suffering long-term symptoms. But long haulers also include those who had moderate, mild or even no COVID-19 symptoms. The problems vary widely and symptoms include chronic fatigue, brain fog (aka COVID brain), depression, shortness of breath, loss of taste and smell, hair loss and more. The signs and symptoms may resolve in a matter of weeks or stubbornly hang on.<\/p>\n<p>A complex condition that defies easy answers requires multiple minds to explore it. That\u2019s the approach of the <a href=\"https:\/\/www.uchealth.org\/today\/covid-19-long-haulers-get-help-at-special-icu-clinic\/\">UCHealth Post-COVID Clinic<\/a>. It brings together specialists in critical care, pulmonology, cardiology, physical medicine and rehabilitation, psychiatry, neurology and more to craft treatment plans that address each patient\u2019s long COVID challenges. To learn more, UCHealth Today spoke with <a href=\"https:\/\/www.uchealth.org\/provider\/sarah-jolley-md\/\">Dr. Sarah Jolley<\/a>, assistant professor of\u00a0<a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/pulmonary\" target=\"_blank\" rel=\"noopener\">Pulmonary Sciences &amp; Critical Care Medicine at the University of Colorado School of Medicine<\/a> on the <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-at-university-of-colorado-anschutz-medical-campus\/\">Anschutz Medical Campus<\/a>. Jolley is the medical director of the clinic.<\/p>\n<h2><strong>Just how prevalent is long COVID?<\/strong><\/h2>\n<p>It\u2019s a complicated question because symptoms after a bout with COVID-19 can be the result of underlying, pre-existing conditions, Jolley said. More on that later. But she estimated that 10-to-20% of patients who contracted COVID-19 experience long COVID, meaning one or more persistent symptoms they had not had prior to infection. \u201cWe\u2019re hoping to see less long COVID after omicron, but we don\u2019t know yet,\u201d Jolley said.<\/p>\n<figure id=\"attachment_62550\" aria-describedby=\"caption-attachment-62550\" style=\"width: 230px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-62550 size-medium\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2022\/02\/16150954\/Post-COVID-Clinic-1-Sarah-Jolley.webp\" alt=\"Dr. Sarah Jolley is the medical director of the UCHealth Post-COVID Clinic, a place where patients get help with their symptoms of long COVID.\" width=\"230\" height=\"300\" srcset=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2022\/02\/16150954\/Post-COVID-Clinic-1-Sarah-Jolley.webp 321w, https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2022\/02\/16150954\/Post-COVID-Clinic-1-Sarah-Jolley-230x300.webp 230w, https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2022\/02\/16150954\/Post-COVID-Clinic-1-Sarah-Jolley-115x150.webp 115w, https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2022\/02\/16150954\/Post-COVID-Clinic-1-Sarah-Jolley-200x261.webp 200w\" sizes=\"auto, (max-width: 230px) 100vw, 230px\" \/><figcaption id=\"caption-attachment-62550\" class=\"wp-caption-text\">Dr. Sarah Jolley is the medical director of the UCHealth Post-COVID Clinic, a place where patients get help with their symptoms of long COVID.<\/figcaption><\/figure>\n<h2><strong>Does a lengthy ICU stay after COVID-19 infection put me at greater risk of experiencing long COVID?<\/strong><\/h2>\n<p>Yes, but people confined to ICUs for long periods are at risk of developing <a id=\"\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6544795\/\" target=\"_blank\" rel=\"noopener\">post-intensive care syndrome (PICS)<\/a>, regardless of their illness or injury, Jolley said. PICS refers to a collection of physical, cognitive and mental health challenges that hang on for long periods after hospitalization \u2013 much like long COVID.<\/p>\n<p>\u201cCOVID-19 ICU survivors are much like those who survive other causes of severe illnesses,\u201d Jolley said. \u201cLong COVID in these cases mirrors PICS, which was described before we had COVID.\u201d<\/p>\n<h2><strong>So if I didn\u2019t get very sick from COVID-19, am I in the clear for long COVID?<\/strong><\/h2>\n<p>No. \u201cWe\u2019ve been seeing patients who had mild to moderate cases of COVID-19,\u201d Jolley said.<\/p>\n<h2><strong>What are the most common symptoms of long COVID?<\/strong><\/h2>\n<p>Jolley listed ongoing shortness of breath, chronic cough and fatigue as three that she often sees. But a host of others appear regularly, including brain fog and confusion; headaches; nerve problems, such as tingling and pain in the extremities; chest pain; and heart palpitations.<\/p>\n<p>Some symptoms are even more confusing and difficult to address, Jolley said. These include persistent loss of taste and smell, hair loss and <a id=\"\" href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/orthostatic-intolerance\" target=\"_blank\" rel=\"noopener\">orthostatic intolerance<\/a>, or dysautonomia, which disrupts the autonomic nervous system \u2013 the one that governs bodily functions like heart rate and blood pressure. One of the many symptoms: dizziness when a person moves from lying down to standing.<\/p>\n<h2><strong>How does the UCHealth Post-COVID Clinic address long COVID concerns?<\/strong><\/h2>\n<p>The clinic is multidisciplinary, which means that specialists work together to address the broad range of long COVID symptoms patients suffer. A nurse navigator screens referrals to understand each patient\u2019s needs upfront, Jolley said.<\/p>\n<p>\u201cBased on that, patients are set up for multidisciplinary visits on the same day for the systems involved,\u201d she said. After a two- to two-and-a-half-hour exam, specialists meet to develop an individualized recovery plan. The approach minimizes testing, prevents multiple visits and sets the patient on a defined treatment course, Jolley explained.<\/p>\n<h2><strong>If I have persistent symptoms after COVID, do I necessarily have long COVID?<\/strong><\/h2>\n<p>No. Jolley said many patients\u2019 post-COVID symptoms may be attributed to underlying conditions, including other viral infections, heart disease, thyroid disease, mood disorders and more.<\/p>\n<p>\u201cMy concern as a physician who sees long COVID patients is there is a tendency for health care providers to attribute anything that happens after COVID to the fact that a patient had COVID,\u201d she said. \u201cBut certainly patients are at risk for all the things they had before COVID. It\u2019s important for care providers to make sure that we\u2019re ruling out all of the non-COVID issues as well.\u201d<\/p>\n<h2><strong>Is there overlap between the symptoms of long COVID and other conditions?<\/strong><\/h2>\n<p>Yes. That\u2019s one of the major difficulties in addressing long COVID, Jolley said. For example, the chronic cough reported by patients who recovered from COVID may be related to pre-existing asthma. At the same time, some COVID patients who have never had asthma may struggle with a persistent cough because the virus inflames their airways, she added. \u201cIt doesn\u2019t seem to predispose them to future asthma,\u201d she noted, although the cough tends to last longer for COVID patients than for patients who recover from the flu or <a id=\"\" href=\"https:\/\/www.cdc.gov\/rsv\/index.html\" target=\"_blank\" rel=\"noopener\">respiratory syncytial virus<\/a>.<div class=\"su-pullquote su-pullquote-align-right\">Learn more about <a href=\"https:\/\/www.uchealth.org\/today\/how-long-is-rsv-contagious-get-facts-about-this-virus\/\">RSV and its symptoms<\/a>.<\/div>\n<h2><strong>Does hair loss for long COVID patients fall in that category?<\/strong><\/h2>\n<p>Yes. Some patients have reported their hair falling out in clumps after recovering from COVID. It\u2019s a problem that \u201ccan be very concerning,\u201d Jolley acknowledged but added that the shedding is usually temporary. She added that the condition, called <a id=\"\" href=\"https:\/\/www.aad.org\/public\/diseases\/hair-loss\/insider\/shedding\" target=\"_blank\" rel=\"noopener\">telogen effluvium<\/a>, can also be traced to thyroid disease, illness with high fever and other causes.<\/p>\n<p>\u201cWe recommend that clinics and primary care providers make sure that there is not an underlying problem that is leading to the hair loss,\u201d Jolley said.<\/p>\n<h2><strong>What do we know about the chronic fatigue that frequently seems to follow long COVID?<\/strong><\/h2>\n<p>This is an important area of study, with a focus on the possibility that long COVID symptoms again overlap with another condition, <a id=\"\" href=\"https:\/\/www.cdc.gov\/me-cfs\/about\/index.html\" target=\"_blank\" rel=\"noopener\">myalgic encephalomyelitis\/chronic fatigue syndrome (ME\/CFS<\/a>), Jolley said. Many of the <a id=\"\" href=\"https:\/\/solvecfs.org\/me-cfs-long-covid\/about-the-disease\/\" target=\"_blank\" rel=\"noopener\">symptoms of ME\/CFS<\/a> mirror those reported by long COVID patients, including sleep problems, joint and muscle pain, brain fog and cognitive impairment. It\u2019s difficult to untangle the source of the symptoms in patients struggling after a COVID bout, Jolley said.<\/p>\n<p>A <a id=\"\" href=\"https:\/\/www.mayoclinicproceedings.org\/article\/S0025-6196(21)00513-9\/fulltext\" target=\"_blank\" rel=\"noopener\">November 2021 paper in the <em>Mayo Clinic Proceedings<\/em><\/a> emphasized the difficulty of providing meaningful clinical help for patients suffering from these symptoms. The authors noted that ME\/CFS was often undiagnosed or inadequately treated long before COVID-19 arrived.<\/p>\n<p>\u201cThese problems are of increasing importance because, after acute COVID-19, a significant percentage of people remain ill for many months with an illness similar to ME\/CFS,\u201d they wrote.<\/p>\n<h2><strong>What can be done to address chronic fatigue for long COVID patients?<\/strong><\/h2>\n<p>Jolley reiterated the importance of identifying, if possible, other underlying causes of the problem. A \u201cconsensus guidance statement\u201d co-authored by <a href=\"https:\/\/www.uchealth.org\/provider\/william-niehaus-md-physical-medicine-and-rehabilitation\/\">Dr. William Niehaus<\/a>, assistant professor of <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/physical-medicine-and-rehabilitation\" target=\"_blank\" rel=\"noopener\">Physical Medicine and Rehabilitation<\/a> at CU (and a provider in the UCHealth Post-Covid Clinic) noted that these include inadequate sleep and nutrition; infectious and autoimmune diseases; and heart, lung and nerve disorders.<\/p>\n<p>The authors offer a series of recommendations to treat fatigue that include developing a phased return-to-activity program; devising strategies to help patients conserve energy; encouraging healthy eating and adequate hydration; and prescribing appropriate medications and herbal and vitamin supplements.<\/p>\n<h2><strong>Are there resources I can use to help myself with long COVID symptoms?<\/strong><\/h2>\n<p>Yes. Jolley said she frequently refers patients to Survivor Corps, a patient advocacy group whose website contains extensive information on long COVID. For patients dealing with loss of taste and smell \u2013 another frequently reported post-COVID symptom \u2013 Jolley recommends the United Kingdom-based <a id=\"\" href=\"https:\/\/www.fifthsense.org.uk\/smell-training\/\" target=\"_blank\" rel=\"noopener\">Fifth Sense<\/a>, which includes tools for self-testing and training. The <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-ear-nose-and-throat-anschutz\/\">UCHealth Ear, Nose and Throat Clinic \u2013 Anschutz Medical Campus<\/a> also offers help with this problem. <a href=\"https:\/\/www.uchealth.org\/today\/how-to-regain-sense-of-taste-and-smell-after-covid-19\/\">Learn about ideas for getting your taste and smell back.<\/a><\/p>\n<p>For patients struggling with the chronic medical and mental health challenges of long COVID, the Psychiatry Consultation for the Medically Complex service, led by <a href=\"https:\/\/www.uchealth.org\/provider\/thida-thant-md\/\">Dr. Thida Thant<\/a>, assistant professor of <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/psychiatry\" target=\"_blank\" rel=\"noopener\">Psychiatry<\/a> at CU, provides a Coping with Illness support group within the <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-psychiatric-services-outpatient-anschutz\/\">UCHealth Outpatient Psychiatric Clinic<\/a>.<\/p>\n<p>\u201cWe find sometimes that our patients are coming from small towns where their docs may not have seen long COVID yet,\u201d Jolley said. \u201cBeing able to connect with someone who is experiencing something similar and saying, \u2018this is real\u2019 can be very meaningful.\u201d<\/p>\n<h2><strong>It&#8217;s been nearly two years since COVID-19 came on the scene. What have we learned about long COVID?<\/strong><\/h2>\n<p>\u201cIn the beginning, there was a thought that long COVID was one singular thing,\u201d Jolley said. \u201cWe\u2019ve learned over time that there are subsets of patients with different constellations of symptoms.\u201d That recognition underscores the importance of taking the multidisciplinary approach of the Post-COVID Clinic.<\/p>\n<p>\u201cAlso, for the most part, patients with long COVID get better over time,\u201d Jolley said. \u201cI don\u2019t think we fully understand it yet, but with enough time, the symptoms do lessen.\u201d<\/p>\n<p>An important and unanswered question with COVID generally and long COVID, in particular, is why the virus affects patients so differently.<\/p>\n<p>\u201cIt suggests there may be different immune profiles among long COVID patients,\u201d Jolley said. One subject of <a id=\"\" href=\"https:\/\/www.livescience.com\/long-covid-four-potential-risk-factors\" target=\"_blank\" rel=\"noopener\">recent research<\/a>, for example, is autoantibodies \u2013 those that target the body\u2019s own tissues, as in rheumatoid arthritis and <a href=\"https:\/\/www.uchealth.org\/diseases-conditions\/lupus\/\">lupus<\/a>. The study of some 300 COVID patients suggests that specific autoantibodies trigger some of the symptoms of long COVID, Jolley said.<\/p>\n<p>The same study and another <a id=\"\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8233978\/\" target=\"_blank\" rel=\"noopener\">published in June 2021<\/a> pointed to a connection between long COVID symptoms and the Epstein-Barr virus, which lies dormant in most of us. The suspicion is that in some people, inflammation triggered by the SARS-CoV-2 virus reactivates the Epstein-Barr virus and leads to long COVID symptoms. These and other studies are probing the bodily mechanisms that underlie long COVID, Jolley said.<\/p>\n<h2><strong>Is more research on long COVID on the horizon?<\/strong><\/h2>\n<p>Yes. Perhaps the most important is a large National Institutes of Health study called <a id=\"\" href=\"https:\/\/recovercovid.org\/about\" target=\"_blank\" rel=\"noopener\">RECOVER<\/a>. The initiative aims to recruit some 17,000 patients across the United States, including long COVID patients, those who recovered without long COVID and healthy controls. University of Colorado Anschutz Medical Campus is part of a consortium with University of Utah, Intermountain Healthcare, University of New Mexico and Denver Health and Hospitals that will be recruiting, enrolling and studying patients.<\/p>\n<p>The RECOVER initiative has already yielded some research contributions. They include a project in which Jolley and a team of CU biostatisticians and data analysts used <a id=\"\" href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2021.10.18.21265168v1?__cf_chl_jschl_tk__=x80imISiDWmRHho0B1qrDkexwtYYvAiSLFVUBpZtN4M-1642564084-0-gaNycGzNCOU\" target=\"_blank\" rel=\"noopener\">machine learning and artificial intelligence to scour the electronic health record<\/a> for patients at risk for long COVID.<\/p>\n<p>\u201cRECOVER should give us a good understanding of the biologic basis for the abnormalities we are seeing after COVID infections,\u201d Jolley said.
