He had always considered himself an average man \u2014 never too concerned about his weight and active enough. He\u2019s now on the path to fitness and attends several different classes a week.<\/figcaption><\/figure>\n<p>What he didn\u2019t want to do was to have to spend money on health care bills \u2014 and neither did his employer.<\/p>\n<p>About three years ago, <a href=\"https:\/\/www.psdschools.org\/careers\/working-at-psd\/benefits-overview\" target=\"_blank\" rel=\"noopener noreferrer\">PSD partnered with UCHealth<\/a> and Associates in Family Medicine to create a program that would provide district employees with an integrated health care option that could help improve employees\u2019 lives while also saving the district in health care costs.<\/p>\n<p>In its first three years, the district figured the program saved about $85,000 in avoided health care claims with its original 56 participants. The program now has more than 200 participants.<\/p>\n<p>\u201cThe increased access to care is huge for our employees,\u201d said Ashley Schwader, PSD wellness manager. \u201cIt has removed barriers and made staying healthy more convenient and affordable.\u201d<\/p>\n<p><a href=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2017\/02\/28144719\/EMPLOYER-corporate-health-and-wellness.pdf\">UCHealth\u2019s Corporate Health and Wellness<\/a> provides several programs directly to employers, said Katie Shilts, program outreach manager for Corporate Health and Wellness.<\/p>\n<p>PSD, a self-insured employer, has aligned with UCHealth\u2019s employee health clinic and lifestyle health and wellness programs. UCHealth also provides an occupational health clinic model. Although these are the three main programs, Shilts said that because each employer\u2019s needs are different, an a la carte design is available to ensure that all needs are met.<\/p>\n<p>\u201cSelf-insured employers benefit by aligning with <a href=\"https:\/\/www.uchealth.org\">UCHealth<\/a> by buying into a clinic model,\u201d she said. \u201cWe can offer variations of on-site and off-site clinics, or like Poudre School District, a near-site clinic so their employees can have easy access.\u201d<\/p>\n<p>Cobb, after being a member of almost every health club in Fort Collins at one time or another, was intrigued by the district\u2019s new program and what it offered him \u2014 for free.<\/p>\n<p>\u201cI would get into these health kicks and join a gym,\u201d he said. \u201cI\u2019m not sure why I wasn\u2019t able to stick with it \u2014 I always had excuses for not going.\u201d<\/p>\n<p>Because of the district\u2019s integrated health plan with UCHealth, Cobb has services available to him through a Lifestyle and Wellness program. Cobb is in his third year of going to the gym regularly and working with UCHealth\u2019s Lifestyle and Wellness team, which includes access to a registered nurse, dietitian and fitness expert. The fitness expert is provided through a partnership between UCHealth and Miramont Lifestyle Fitness.<\/p>\n<p>\u201cI think it helps having someone to talk to. I like having that person to go to if I have questions,\u201d Cobb said. \u201cI\u2019ve been supported all along the way \u2014 you\u2019ve got people looking after you, and I think that is important.&#8221;<\/p>\n<p>That personalized approach is what sets UCHealth\u2019s program apart from others, Shilts said.<\/p>\n<p>\u201cIt\u2019s a disease management program on steroids,\u201d she said. \u201cMost programs are passive, but ours meets face-to-face. Employees even sign an agreement to follow through with the program. It\u2019s very outfacing.\u201d<\/p>\n<p>Lifestyle Health Manager and registered nurse Laura Dvorak said the one-on-one support is what most clients she works with say they find most beneficial.<\/p>\n<p>Her clients are either referred to her by a physician, or like Cobb, are self-referred. She meets with both district employees and their dependents who are on the district\u2019s health insurance plan.<\/p>\n<p>First, her team completes a health, diet and fitness assessment with the new client.<\/p>\n<p>\u201cWe look at all aspects of their health,\u201d Dvorak said. \u201cWe talk about their current concerns, their knowledge about healthy choices, their habits \u2014 we get a picture of what their lifestyle is now so that we can create a plan for moving forward.\u201d<\/p>\n<p>Cobb learned from his first fitness evaluation that he had the physical health of a 73-year-old \u2014 he was only 63 at the time. But it took him only six months of following the plan the team set out for him to drop that physical health age to 57.<\/p>\n<p>Cobb, who had also been diagnosed with Type 2 diabetes two years before starting the district\u2019s program, knew he needed to continue to address his chronic illness and saw the integrated health plan as a perfect opportunity. And now, his doctor has taken him off his diabetes medication.<\/p>\n<p>\u201cMy health has kept me going [with the program],\u201d he said. \u201cI no longer have to take medication for my diabetes because of exercising, I\u2019m more pleased with my appearance, and I\u2019m now active with people my own age. I\u2019m no longer the one falling behind, but I\u2019m leading my peers. I feel I\u2019m more fit and able to enjoy life. I\u2019m more confident in myself.\u201d<\/p>\n<p>Cobb said he still stumbles occasionally, but he has the Lifestyle team right there to encourage him to get up. Recently, he\u2019s found he is struggling with his diet, so he\u2019s scheduled a time to meet with UCHealth\u2019s Corporate Health and Wellness registered dietitian Brooke Floerke.<\/p>\n<p>He\u2019s even become what he calls a \u201cyoga believer,\u201d he joked.<\/p>\n<p>\u201cIt could have been another program that they offer, but I needed flexibility \u2014 those were my worst scores on my evaluation,\u201d Cobb said. \u201cIt\u2019s not my best area, but it\u2019s been my biggest improvement.\u201d<\/p>\n<p>Because a small share of the population accounts for most of the health care spending in the United States, and because 75 percent of those costs come from preventable diseases, such as diabetes and heart disease, it makes sense that employers provide accessible opportunities for their workers to get and stay healthy, Shilts explained.<\/p>\n<p>That\u2019s been the case for Cobb and many others in the program, Schwader said.<\/p>\n<p>\u201cThis [program] has been my fountain of youth,\u201d Cobb said. \u201cI plan to enjoy my later years of life.\u201d<\/p>\n","protected":false},"excerpt":{"rendered":"<p>John Cobb felt as though he was getting old and flabby.
