Photo courtesy of Abby Zamora.<\/figcaption><\/figure>\n<p>When she reflects on her experience having kids, Abby Zamora chuckles and admits many women, and even traditionally minded men, probably think she\u2019s crazy.<\/p>\n<p>\u201cI love pregnancy, labor and delivery,\u201d she says. \u201cIf you love something, you do it.\u201d<\/p>\n<p>She\u2019s had ample opportunity to test that love. Zamora, 36, and husband Steven have three children of their own \u2013 daughters 7 and 4, and a son, 5. She\u2019s also been a gestational surrogate for intended parents. Zamora was in labor for 48 hours with her boy, one of her three \u201ckeepers,\u201d as she put it, and finally chose an epidural to manage the pain only in the last two hours. She had little pain with her first daughter, but was in labor for 36 hours and delivered after three hours of pushing. Those kids both arrived past their due dates. Photo courtesy of Abby Zamora.<\/figcaption><\/figure>\n<p>Aside from the unswerving supportive presence of Steven, one factor unites Zamora\u2019s deliveries. They all happened under the care of certified nurse-midwives at <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-university-of-colorado-hospital-uch\/\">UCHealth University of Colorado Hospital<\/a> on the <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-at-university-of-colorado-anschutz-medical-campus\/\">Anschutz Medical Campus<\/a>. These midwives are part of the <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-center-for-midwifery-anschutz\/\">UCHealth Center for Midwifery<\/a>, which along with other sites provides care from certified nurse-midwives across the metro area (see box).<\/p>\n<p>When the attorneys for the couple whose unborn child Zamora now carries told her they wanted her to deliver with an obstetrician, she, in turn, delivered a simple message. \u201cI said, \u2018It\u2019s my midwives or I\u2019m walking,\u2019\u201d she recalled, and held her ground successfully.<\/p>\n<p>The mini-confrontation begs a couple of questions. Just who are these nurse-midwives and why should there be resistance to them? It\u2019s an especially opportune time to pose the questions. October 4 marks the start of <a href=\"https:\/\/www.midwife.org\/National-Midwifery-Week\" target=\"_blank\" rel=\"noopener noreferrer\">National Midwifery Week<\/a>. Photo courtesy of Abby Zamora.<\/figcaption><\/figure>\n<p>As to what they are: Certified nurse-midwives are registered nurses who complete master\u2019s-level midwife education programs approved by the national <a href=\"https:\/\/www.midwife.org\/accreditation\" target=\"_blank\" rel=\"noopener noreferrer\">Accreditation Commission for Midwifery Education<\/a>. They receive certification after passing an exam through the <a href=\"https:\/\/www.amcbmidwife.org\/amcb-certification\" target=\"_blank\" rel=\"noopener noreferrer\">American Midwifery Certification Board<\/a>.<\/p>\n<p>Certified nurse midwives assist during childbirth, but they provide a <a href=\"https:\/\/www.uchealth.org\/services\/womens-care\/midwifery-services\/\">range of services encompassing women\u2019s health<\/a> before, during and after pregnancy. In so doing, they \u201caugment\u201d the care provided by maternal and fetal medicine specialists, obstetricians, and family medicine physicians, said <a href=\"https:\/\/www.uchealth.org\/provider\/jessica-anderson-msn\/\">Jessica Anderson<\/a>, director of Midwifery Services\u00a0and associate professor of Clinical Practice at the <a href=\"https:\/\/nursing.cuanschutz.edu\/about\" target=\"_blank\" rel=\"noopener noreferrer\">University of Colorado College of Nursing<\/a> (CU Nursing). Anderson noted that 2020 marks the <a href=\"https:\/\/news.cuanschutz.edu\/nursing\/nurse-midwifery-program-gives-birth-to-forty-years-of-firsts\" target=\"_blank\" rel=\"noopener noreferrer\">40<sup>th<\/sup> year of the midwife education program at CU Nursing<\/a>, which was recently named one of the top\u00a0 midwifery programs in the country.<\/p>\n<p>\u201cWe are a unique profession and serve different purposes,\u201d said Anderson, who delivered Zamora\u2019s third keeper. For example, nurse-midwives refer their high-risk pregnancy patients to physician specialists, but physicians also refer low-risk patients to midwives.<\/p>\n<p>\u201cPatients want options to choose what is best for them,\u201d Anderson said. \u201cThey also have the ability to move around the spectrum of care based on their health care needs. Photo by UCHealth.<\/figcaption><\/figure>\n<p>\u201cWhen you mention nurse-midwives, people may think of a hippie chick coming to your door, giving everyone a shot of vodka and you have a baby in your living room,\u201d she chuckled. But she admits her decision in 2012 to have a nurse-midwife assist in her delivery concerned even husband Steven at first. \u201cEverybody\u2019s stigma is that you\u2019ll have a home birth, unmedicated, with someone with no medical training,\u201d Zamora said.<\/p>\n<p>That\u2019s far from the reality. All women who receive care from nurse-midwives at UCHealth and the University of Colorado deliver at University of Colorado Hospital, where higher-level care is readily available if it\u2019s needed. (About 94% of certified nurse midwife-assisted births nationwide occur in hospitals.) That fact, and a few visits with Abby\u2019s nurse-midwives, quickly allayed Steven\u2019s fears and, later, the concerns of the attorneys for the intended parents of her current surrogacy.<\/p>\n<p>Why choose a certified nurse midwife-assisted birth rather than an obstetrician? It\u2019s still a relatively uncommon choice, representing only about 9% of births in the United States. Anderson said midwives use their nursing and midwifery skills to create a \u201cpersonalized\u201d experience for women that focuses on meeting their individual needs and expectations for childbirth and their health as a whole.<\/p>\n<h3><strong>Individualized approach<\/strong><\/h3>\n<p>Zamora said she appreciated the \u201cquiet strength\u201d that her nurse-midwives projected during each of her births. They stood ready to call in additional medical care if a problem arose, like the baby\u2019s heart rate decelerating with contractions. With her second biological child, her prolonged labor led her midwife to warn she might need induced labor or a C-section instead of the natural birth she wanted. But after she agreed to medications to help her sleep, she emerged \u201cre-energized\u201d to get through the rest of her labor. She asked for an epidural, her midwife checked her progress and approved, and Zamora delivered her baby about two hours later.<\/p>\n<p><strong><div class=\"su-callout-box col-xs-12 col-sm-6 right\" style=\"background-color:#dce4e7; color:#2e3b44;\"><\/strong><\/p>\n<p>Sites for midwifery services in the UCHealth\/University of Colorado system:<\/p>\n<ul>\n<li><a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-center-for-midwifery-anschutz\/\">UCHealth Center for Midwifery \u2013 Anschutz Medical Campus<\/a><\/li>\n<li><a href=\"https:\/\/nursing.cuanschutz.edu\/patient-care\/nurse-midwifery-locations\/cfm-lone-tree\">UCHealth Center for Midwifery at Lone Tree Medical Center<\/a><\/li>\n<li><a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-birth-center-highlands-ranch-hospital\/\">UCHealth Birth Center \u2013 Highlands Ranch Hospital<\/a><\/li>\n<li><a href=\"http:\/\/www.cucfmlongmont.com\/\">CU Center for Midwifery \u2013 Longmont<\/a><\/li>\n<li><a href=\"https:\/\/nursing.cuanschutz.edu\/patient-care\/nurse-midwifery-locations\/university-nurse-midwives\">University Nurse Midwives Faculty Practice at Anschutz Outpatient Pavilion<\/a><\/li>\n<li><a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-center-for-midwifery-lowry\/\">UCHealth Center for Midwifery at Lowry<\/a><\/li>\n<li><a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-center-for-midwifery-boulder\/\">UCHealth Center for Midwifery \u2013 Boulder<\/a><strong><\/div><\/strong><\/li>\n<\/ul>\n<p>The example illustrates her midwives\u2019 consistent commitment to letting her listen to her own body and make the calls on how her labor should proceed, she said.<\/p>\n<p>\u201cThey let me labor and do what I needed to do and what I felt I was comfortable with,\u201d Zamora said. She could move from the tub used for <a href=\"https:\/\/www.uchealth.org\/services\/womens-care\/midwifery-services\/water-birth\/#:~:text=As%20part%20of%20our%20comprehensive,Colorado%20Hospital%20in%20metro%20Denver.\">water births<\/a> to a birthing ball to the bed \u2013 anywhere that made her most comfortable while the midwife and her team \u201clet it roll\u201d and did whatever they could to help her.<\/p>\n<p>\u201cThey made me feel I was in control of the situation,\u201d Zamora said.<\/p>\n<h3><strong>Midwife-mom partnership<\/strong><\/h3>\n<p>Anderson characterized the relationship between mother and midwife as a \u201cpartnership\u201d based on \u201cshared decision making\u201d that allows women \u201cto make decisions that are best for them.\u201d The idea is to find \u201ca happy spot\u201d where higher-level interventions are available but used \u201cat the right place, at the right time and with the right patient.\u201d<\/p>\n<p>That might mean, for example, needing to go to a cesarean delivery because of concerns for the baby\u2019s heart. Or it might mean support interventions like water labor to avoid an epidural if the mother doesn\u2019t request one.<\/p>\n<p>There is some evidence that this approach has pay-offs that extend beyond patient satisfaction. Some studies show lower rates of C-sections for women cared for by nurse midwives, resulting in significant cost savings. Anderson noted that while epidural pain medication is always an option for women, a \u201csignificant chunk\u201d of women assisted by certified nurse-midwives don\u2019t choose that option, another resource-saving. <a href=\"http:\/\/www.midwife.org\/acnm\/files\/ccLibraryFiles\/Filename\/000000004184\/Midwifery-Evidence-Based-Practice-March-2013.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">A 2012 summary by the American College of Nurse Midwives<\/a> outlined other benefits of certified nurse-midwife care, including reduced use of anesthesia, higher rates of breastfeeding and fewer instances of complications, like perineal tears during childbirth.<\/p>\n<h3><strong>Wanted: more nurse-midwives<\/strong><\/h3>\n<p>These benefits and ongoing pressures on health care resources point to the need to expand the number of midwives and the places they provide care, Anderson said. It\u2019s no easy task to accomplish.<\/p>\n<p>\u201cIt costs money to start practices,\u201d she said. \u201cWe need institutional support and we need to find ways to engage more physicians to hire and collaborate with midwives in their practices.\u201d<\/p>\n<p>Another important challenge, Anderson added, is educating enough midwives to meet the needs of the communities they care for.<\/p>\n<p>\u201cWe think part of the answer is to expand the midwifery workforce and support more midwives of color,\u201d Anderson said. She noted that the American College of Nurse Midwives issued a <a href=\"https:\/\/www.midwife.org\/acnm\/files\/acnmlibrarydata\/uploadfilename\/000000000315\/PS-Racism%20and%20Racial%20Bias%20FINAL%20to%20ACNM%2026-Oct-19.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">position statement<\/a> on racism and racial bias that in part commits to \u201cincreasing racial and ethnic diversity within the profession.\u201d<\/p>\n<p>Anderson said work is underway on the Anschutz Medical Campus to find ways to \u201crecruit more diversity into our faculty and educational programs. We want to impact the future and have programs that are sustainable. Ultimately, we need more midwives.\u201d<\/p>\n<p>In the meantime, UCHealth continues to expand the number of practices offering certified nurse-midwife services within its system. The newest site, at UCHealth Highlands Ranch Hospital, opened in March. Despite the challenges of the COVID-19 pandemic, the midwife practice there is \u201ctalking about adding more clinic days,\u201d Anderson said. That would add to the nearly 20,000 visits and more than 1,500 births handled by UCHealth midwives in 2019, she noted.<\/p>\n<h3><strong>Important option for women<\/strong><\/h3>\n<figure id=\"attachment_34968\" aria-describedby=\"caption-attachment-34968\" style=\"width: 400px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-34968\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/10\/07203636\/Baby-bump-tiny.webp\" alt=\"Abby Zamora has loved having certified nurse-midwives for all of her births. She continues to meet regularly with a rotating cast of midwives for her pregnancy care, which continues to six weeks after she gives birth. But she\u2019s now taken that a step further and has moved all her women\u2019s health care to the Center for Midwifery.<\/p>\n<p>After four previous births, visits with the midwives for her current pregnancy are \u201clike coffee with a friend,\u201d Zamora said. She\u2019s still deciding on her final birth plan. She had a water birth for her first surrogacy but hasn\u2019t yet decided if she will do that again.
