With plenty of warm, sunny days and high elevations that increase the intensity of the sun\u2019s ultraviolet (UV) rays, the state leads the nation in an unwanted category: the per-capita rate of <a href=\"https:\/\/www.uchealth.org\/diseases-conditions\/skin-cancer\/\">skin cancer<\/a>.<\/p>\n<p>That\u2019s not good news. But there is a bright spot. It\u2019s called Mohs micrographic surgery, a delicate procedure to remove cancerous tissue in patients with some skin cancers, including two types \u2013 basal cell and squamous cell carcinomas \u2013 that arise in the epidermis, or outer layer of the skin. The procedure will be available to patients at the new <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-cherry-creek-medical-center\/\">UCHealth Cherry Creek Medical Center<\/a> at 100 Cook Street in Denver.<\/p>\n<h3><strong>Mohs surgery has very high success rate<\/strong><\/h3>\n<figure id=\"attachment_34153\" aria-describedby=\"caption-attachment-34153\" style=\"width: 166px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-34153 size-full\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/09\/07083140\/Dr.MishaMiller-e1599489779972.webp\" alt=\"Dr. Misha Miller, who is part of the Mohs surgery team at Cherry Creek.\" width=\"166\" height=\"184\" srcset=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/09\/07083140\/Dr.MishaMiller-e1599489779972.webp 166w, https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/09\/07083140\/Dr.MishaMiller-e1599489779972-135x150.webp 135w\" sizes=\"auto, (max-width: 166px) 100vw, 166px\" \/><figcaption id=\"caption-attachment-34153\" class=\"wp-caption-text\">Dr. Misha Miller<\/figcaption><\/figure>\n<p>\u201cMohs surgery is one of the most effective ways of extirpating tumors of skin cancers,\u201d said <a href=\"https:\/\/www.uchealth.org\/provider\/misha-miller\/\">Dr. Misha Miller<\/a>, who practices at the <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-dermatology-clinic-anschutz\/\">UCHealth Dermatology Clinic on the Anschutz Medical Campus<\/a>, and is an assistant professor in the <a href=\"https:\/\/medschool.cuanschutz.edu\/dermatology\" target=\"_blank\" rel=\"noopener noreferrer\">Department of Dermatology<\/a> at the <a href=\"https:\/\/medschool.cuanschutz.edu\/dermatology\" target=\"_blank\" rel=\"noopener noreferrer\">University of Colorado School of Medicine<\/a>.<\/p>\n<p>The procedure achieves success rates between 98% and 99% for basal and squamous cell carcinomas, added Miller, who will be in charge of Mohs surgeries at the new Cherry Creek facility.<\/p>\n<p>Performing Mohs surgeries requires a specialized set of skills, which Miller built during fellowship training in the procedure at the CU School of Medicine. These include surgery, pathology and wound reconstruction. All three come into play during the procedure.<\/p>\n<h3><strong>Mohs surgery part of a multi-stage treatment<\/strong><\/h3>\n<div class=\"su-callout-box col-xs-6 col-sm-6 right\" style=\"background-color:#dce4e7; color:#2e3b44;\">\n<p><strong>Read <a href=\"https:\/\/www.uchealth.org\/today\/tag\/UCHealth-Cherry-Creek-Medical-Center\/\">more stories<\/a> about providers, patients and services at the <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-cherry-creek-medical-center\/\">UCHealth Cherry Creek Medical Center<\/a><\/strong><\/p>\n<\/div>\n<p>Most patients get local anesthesia for the outpatient procedure. Miller then uses a scalpel to carefully cut a thin layer of tissue and a narrow rim of normal-appearing skin to remove a cancerous lesion. After doing so, she prepares the tissue she removed. The Mohs Laboratory then processes the tissue into slides. Putting on her pathologist\u2019s hat, Dr. Miller examines the tissue under a microscope for lingering cancer cells. If she finds them, she removes another thin layer and examines the results. The process continues until the area is cancer-free.<\/p>\n<p>Most patients require one or two \u201cpasses\u201d with the scalpel, Dr. Miller said. She also takes care to remove as little healthy tissue as possible during the procedure.<\/p>\n<p>\u201cI can cut out a skin cancer with only a tiny rim of normal tissue taken,\u201d Dr. Miller said.<\/p>\n<h3><strong>Repairing the skin<\/strong><\/h3>\n<figure id=\"attachment_34154\" aria-describedby=\"caption-attachment-34154\" style=\"width: 194px\" class=\"wp-caption alignleft\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-34154 size-full\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/09\/07083218\/Dr.AdamTerella.webp\" alt=\"Dr. Adam Terella, who is part of the Mohs surgery team at Cherry Creek.\" width=\"194\" height=\"248\" srcset=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/09\/07083218\/Dr.AdamTerella.webp 194w, https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/09\/07083218\/Dr.AdamTerella-117x150.webp 117w\" sizes=\"auto, (max-width: 194px) 100vw, 194px\" \/><figcaption id=\"caption-attachment-34154\" class=\"wp-caption-text\">Dr. Adam Terella<\/figcaption><\/figure>\n<p>The final stage involves reconstructing the wound left by the surgery. Miller said she handles about 90% of reconstructions herself, using stitches, skin grafts taken from another area of the patient\u2019s body, or skin flaps taken from an area next to the wound. She also collaborates with <a href=\"https:\/\/www.uchealth.org\/services\/ear-nose-throat-care\/\">UCHealth ear, nose and throat<\/a> physician <a href=\"https:\/\/www.uchealth.org\/provider\/adam-terella-md-otolaryngology\/\">Dr. Adam Terella<\/a>, who has specialized training in plastic and facial reconstruction surgery.<\/p>\n<p>At the Cherry Creek Medical Center, Miller and Terella will act as the reconstructive team for cases involving the head and neck. For cases that involve the eyes, <a href=\"https:\/\/www.uchealth.org\/services\/eye-care\/oculofacial-and-orbital-plastic-surgery\/\">UCHealth oculoplastic surgeons<\/a> Drs. <a href=\"https:\/\/www.uchealth.org\/provider\/eric-hink-md-ophthalmology\/\">Eric Hink<\/a> and <a href=\"https:\/\/www.uchealth.org\/provider\/sophie-liao-md-ophthalmology\/\">Sophie Liao<\/a> will complete the reconstruction.<\/p>\n<h3><strong>Catching skin cancer early<\/strong><\/h3>\n<figure id=\"attachment_34155\" aria-describedby=\"caption-attachment-34155\" style=\"width: 195px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-34155 size-full\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/09\/07083258\/Dr.EricHink.webp\" alt=\"Dr. Eric Hink, who is part of the Mohs surgery team at Cherry Creek.\" width=\"195\" height=\"248\" srcset=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/09\/07083258\/Dr.EricHink.webp 195w, https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/09\/07083258\/Dr.EricHink-118x150.webp 118w\" sizes=\"auto, (max-width: 195px) 100vw, 195px\" \/><figcaption id=\"caption-attachment-34155\" class=\"wp-caption-text\">Dr. Eric Hink<\/figcaption><\/figure>\n<p>As with all procedures, a good deal of the success of Mohs surgery relies on early detection. Miller stressed the importance of patients recognizing the warning signs of basal cell and squamous cell carcinomas and contacting a dermatologist about suspicious lesions.<\/p>\n<p>Basal cell carcinoma is the number-one skin cancer. It very rarely metastasizes, or spreads, but left untreated, it will invade and destroy local tissue, leaving deeper and wider damage as it advances, Miller said. The most prominent signs of the cancer are sores that won\u2019t heal and may bleed. These look like pearly-pink bumps or marks that resemble scars.<\/p>\n<p>\u201cIf the lesion bleeds and won\u2019t go away or you have a scar with no memory of injury, you should definitely see a dermatologist,\u201d Miller said.<\/p>\n<figure id=\"attachment_34156\" aria-describedby=\"caption-attachment-34156\" style=\"width: 188px\" class=\"wp-caption alignleft\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-34156 size-full\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/09\/07083338\/Dr.SophieLiao.webp\" alt=\"Dr. Sophie Liao, who is part of the Mohs surgery team at Cherry Creek.\" width=\"188\" height=\"250\" srcset=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/09\/07083338\/Dr.SophieLiao.webp 188w, https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2020\/09\/07083338\/Dr.SophieLiao-113x150.webp 113w\" sizes=\"auto, (max-width: 188px) 100vw, 188px\" \/><figcaption id=\"caption-attachment-34156\" class=\"wp-caption-text\">Dr. Sophie Liao<\/figcaption><\/figure>\n<p>Squamous cell carcinomas are less common than the basal cell variety, said Miller, adding that they are more likely to spread, although that\u2019s still rare. They look like a red, crusty bump on the skin and sometimes hurt when they get a bump or pressure \u2013 a sign that the cancer has moved into the nerves.<\/p>\n<p>The most deadly form of skin cancer is melanoma. It springs from the melanocytes, the cells that give us our skin pigment. Miller said she rarely uses Mohs surgery to treat melanoma, but emphasized how important it is to recognize its signs, which include dark, irregularly shaped patches of skin. People should take particular care to contact a dermatologist if a patch is bigger than 6 millimeters, gets larger and darker and begins itching or tingling, Miller cautioned.<\/p>\n<h3><strong>More treatment opportunities at Cherry Creek<\/strong><\/h3>\n<p>Basal cell and squamous cell carcinomas are largely the result of skin damage people sustained over years, which helps to account for the fact that most of her Mohs surgery patients are in their 60s or older, Miller said. The opening of the Cherry Creek Medical Center offers new opportunities to extend the procedure to more patients. It\u2019s an additional site, closer to home for many, of course. Beyond that, Dr. Miller hopes to expand her work with Dr. Terella.<\/p>\n<p>Today, she noted, most reconstruction cases that involve him aren\u2019t scheduled the same day of surgery. At the new facility, she and Terella plan to work \u201cin tandem\u201d more frequently, with him taking care of reconstruction the same day as surgery. That will free her to move on to the next patient.<\/p>\n<p>\u201cI\u2019ll do Mohs while he does reconstruction,\u201d Miller said. \u201cThat will enable us to care for more patients.\u201d<\/p>\n<p>That kind of partnership has long been a hallmark of the multidisciplinary care patients receive at UCHealth hospitals and clinics.
