{"id":16748,"date":"2018-06-27T08:30:59","date_gmt":"2018-06-27T14:30:59","guid":{"rendered":"https:\/\/www.uchealth.org\/today\/?p=16748"},"modified":"2022-11-16T11:40:34","modified_gmt":"2022-11-16T18:40:34","slug":"skillful-solution-to-a-heart-stopping-condition","status":"publish","type":"post","link":"https:\/\/www.uchealth.org\/today\/skillful-solution-to-a-heart-stopping-condition\/","title":{"rendered":"Skillful solution to a heart-stopping condition"},"content":{"rendered":"<div style=\"margin-top: 0px; margin-bottom: 0px;\" class=\"sharethis-inline-share-buttons\" ><\/div><p>One of the first things Amy Velisek did when she moved to Colorado to be closer to her newborn twin grandchildren was to buy the book \u201cHikes Around Fort Collins.\u201d No sooner did she open the book, she learned the adventures she read about could kill her.<\/p>\n<p>\u201cTwo major catastrophes can occur with an aortic aneurysm,\u201d said<a href=\"https:\/\/www.uchealth.org\/provider\/mark-d-guadagnoli-md-thoracic-and-cardiac-surgery\/\"> Dr. Mark Guadagnoli<\/a>, cardiothoracic surgeon with <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-heart-center-vascular-clinic-at-medical-center-of-the-rockies\/\">UCHealth Heart and Vascular Clinic in Loveland.<\/a><\/p>\n<p>An aortic aneurysm is an enlargement of the largest blood vessel of the body, the aorta. Aneurysms can rupture, causing internal bleeding, or they can dissect, which is a tear and separation of the layers of the aorta. Mortality rates for both are high, he said.<\/p>\n<h3>Just a matter of time<\/h3>\n<p>&#8220;Amy was exposed to constant risk going forward,\u201d Guadagnoli said. \u201cHer aneurysm was approaching a dangerous size, and being a very physically active person, she really couldn\u2019t safely do a lot of the things she loved anymore.\u201d<\/p>\n<div class=\"su-callout-box col-xs-12 col-sm-6 right\" style=\"background-color:#dce4e7; color:#2e3b44;\">\n<p>The weakening of the aortic wall can happen for numerous reasons, including genetic diseases or lifestyle factors such as high blood pressure or high cholesterol. Symptoms, however, do not usually arise.<\/p>\n<p>\u201cThat\u2019s the problem,\u201d said <a href=\"https:\/\/www.uchealth.org\/provider\/mark-d-guadagnoli-md-thoracic-and-cardiac-surgery\/\">Dr. Mark Guadagnoli<\/a>, cardiothoracic surgeon with UCHealth Heart and Vascular Clinic in Loveland. \u201cPeople walk around with an aneurysm and don\u2019t know it. The only way to detect it is a screening examination.\u201d<\/p>\n<p>The economics of screening tests are difficult and are not covered routinely by insurance. Most insurance does not cover chest examinations \u2014 such as an ultrasound, CAT scan or echocardiogram \u2014 as part of a health screening. Medicare does cover abdominal ultrasounds to screen for abdominal aneurysms starting at age 65, which Guadagnoli recommends. But chest aneurysms are mostly discovered incidentally on tests done for other reasons, such as to determine a cause for high blood pressure, as was the case with Velisek.<\/p>\n<\/div>\n<p>The aorta is like a hydraulic line that comes out of the heart, Guadagnoli explained. Every time the heart beats, it accepts the blood that\u2019s being pumped by stretching and relaxing. \u201cIf the heart is the engine, then the aorta is the transmission,\u201d he said. And with roughly 31.5 million heart beats per year, that transmission line is working hard.<\/p>\n<p>An aortic aneurysm occurs when the aorta\u2019s wall begins to weaken and loses elasticity. It then starts to balloon outward; the wall deteriorates and the damaged tissue grows in size, increasing the risk for a rupture or <a href=\"https:\/\/www.uchealth.org\/today\/coming-together-save-mom-baby\/\">dissection<\/a>.<\/p>\n<p>Velisek knew of her condition before moving to Colorado and had been monitoring it for a few years. But because the aneurysm had grown, Guadagnoli recommended surgery \u2014 and because her aneurysm involved the aortic root but not the valve, she was a candidate for a newer approach to that surgery.<\/p>\n<h3>Valve-sparing aortic root procedure (VSAR)<\/h3>\n<p>Although it has been in development for many years, the valve-sparing root procedure is still not widely available except for centers performing more complex aortic root surgery, such as UCHealth, according to Guadagnoli.<\/p>\n<p>The aortic valve is contained within the aortic root and are intimately related, he explained. A valve-sparing aortic root procedure preserves the heart valve during the aneurysm repair as opposed to the traditional procedure that sacrifices and replaces the valve. \u201cThere is nothing like one\u2019s original equipment,\u201d Guadagnoli said<\/p>\n<p>He stressed that heart valve replacements are lifesaving, when necessary, but when the valve is healthy, then this procedure is a great option.<\/p>\n<p>\u201cShe got the best option there is,\u201d he said. \u201cHer aneurysm is repaired, and she has her original heart valve, which should last her the rest of her life.\u201d<\/p>\n<figure id=\"attachment_16749\" aria-describedby=\"caption-attachment-16749\" style=\"width: 200px\" class=\"wp-caption alignleft\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-16749\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2018\/06\/26140346\/Amy-and-daughter1200.webp\" alt=\"\" width=\"200\" height=\"300\" srcset=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2018\/06\/26140346\/Amy-and-daughter1200.webp 668w, https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2018\/06\/26140346\/Amy-and-daughter1200-200x299.webp 200w, https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2018\/06\/26140346\/Amy-and-daughter1200-100x150.webp 100w\" sizes=\"auto, (max-width: 200px) 100vw, 200px\" \/><figcaption id=\"caption-attachment-16749\" class=\"wp-caption-text\">Amy, right, and her daughter in Colorado.<\/figcaption><\/figure>\n<h3>Making the decision<\/h3>\n<p>It wasn\u2019t that cut and dried for Velisek. She struggled with the thought that she\u2019d electively choose to undergo open-heart surgery. The surgery is challenging, and recovery could potentially take a few months. But the benefits outweighed the risks for Velisek.<\/p>\n<p>More than 98 percent of patients who undergo this surgery electively fully recover, whereas most patients presenting with rupture or dissection are facing a dangerous situation with a high-risk of lethality. And Velisek and her family agreed that doing nothing made them all uneasy. And then there was the fact that she wasn\u2019t allowed to be as active as she\u2019d always been.<\/p>\n<p>\u201cNow my life is being restricted by something that can be fixed,\u201d she recalled thinking.<\/p>\n<p>Her daughter helped her schedule the surgery for four weeks later, on April 16, 2018, at UCHealth Heart and Vascular Center &#8211; Medical Center of the Rockies.<\/p>\n<h3>The road to recovery<\/h3>\n<p>After 10 days in the hospital, Velisek was released and began her recovery with help from many visiting family members and a home nurse who\u2019d visit weekly. After three weeks, she started her recovery program \u2014 one hour, three days per week at UCHealth Cardiac Rehabilitation \u2013 Harmony Campus in Fort Collins.<\/p>\n<p>She\u2019ll admit that the first day of rehab she was in tears because of her limitations.
