This is your personal patient portal to request prescription refills, view your medical records or test results and to contact our office. <\/div>\n\n\n\n<p class=\"wp-block-paragraph\">Referring physicians: please call <a href=\"tel:9708752708\">970.875.2708<\/a> to request an appointment with our scheduler.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">While we welcome self-referrals, we kindly request supporting records for review ahead of scheduling an appointment in order to ensure that we are able to deliver personalized care in a timely fashion.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Online scheduling is currently not available.
