Are you overweight? Have you been told your blood sugar is “running a little high”? The time is now for you to make lifestyle changes to prevent or delay type 2 diabetes.
The Diabetes Prevention Program (DPP) at UCHealth in Northern Colorado at Poudre Valley Hospital, Medical Center of the Rockies, and Greeley Hospital helps adults at high risk of developing type 2 diabetes prevent or delay the disease process. Research by the National Institutes of Health has shown that this program can reduce the number of new cases of type 2 diabetes by 58% and 71% in adults over the age of 60. Our program has achieved and maintains full recognition through the CDC led National Diabetes Prevention Program and uses their approved curriculum.
Participants meet with a registered dietitian, who is also a trained DPP lifestyle coach, for one year in 24 one hour sessions in a supportive group environment. You will learn how to change your eating behaviors, increase your activity, and take control of your habits for healthier living.
Covered by Medicare and some commercial insurance companies. We also offer a self-pay rate.
Pre-qualification and pre-registration is required. Call Teresa Reichert at 970.495.8777 or complete this form and you will be contacted to complete pre-qualification questions or for more information.
