The goal of this study is to test whether a new program improves symptoms, functioning, and quality of life in people with chronic heart failure. The program targets common symptoms, including fatigue, shortness of breath, depression, anxiety and pain; helps patients adjust to medical illness; and provides resources to family caregivers. The program integrates this care into ongoing chronic illness care.

Eligibility and Other Participant Information

To be INCLUDED in the study, patients must meet EACH of the following criteria:

1. Age 18 years of age or older
2. Able to read and understand English with consistent access to a telephone
3. Have a primary care or other provider who is willing to facilitate intervention medical recommendations
4. A diagnosis of heart failure and several heart failure-specific criteria
5. Bothered by at least one target symptom:
    a. Pain
    b. Depression
    c. Fatigue
    d. Breathlessness
Exclusion Criteria

Patients will be EXCLUDED if they meet ONE of the following:

1. Previous diagnosis of dementia
2. Active substance abuse or dependence
3. Diagnosis of bipolar disorder or schizophrenia
4. Comorbid metastatic cancer
5. Nursing home resident
6. Heart Transplant Recipient
7. LVAD recipient
