At UCHealth Primary Care – Craig, we offer the best in primary care services. Our highly-trained and compassionate staff can help you obtain and maintain optimal health.
We aim to care for the whole person, and that includes partnering with patients and their families to identify and understand each patient’s unique needs. From routine physicals to behavioral health services to maintenance of chronic conditions like diabetes or cholesterol management, our providers take the time to listen to your concerns and to develop a plan for care with you.
Primary care services
Adult care
As your patient-centered medical home, we provide wellness exams, chronic disease management, and geriatric medicine.
Pediatric care
Our providers offer pediatric care, well-child checks, immunizations, episodic events, injury care, and school physicals.
Sports physicals
We offer sports physicals. Appointments can be scheduled anytime. Bring your child’s school physical form, insurance card, and immunization records. School physicals take approximately 15-30 minutes and cost $30.
Impact testing is not performed at this location, but is offered at the UCHealth SportsMed Clinic at UCHealth Yampa Valley Medical Center in Steamboat Springs.
DOT physicals
We offer Department of Transportation (DOT) physicals, following strict guidelines mandated by the Federal Motor Carrier Safety Administration (FMCSA), ensuring that commercial drivers and other safety-sensitive employees are in good health to work safely.
Worker’s comp
We coordinate work injury care with medical home management, working with individuals and businesses. Employee drug screenings are available.
Women’s health
Personalized women’s health services are offered by female providers. Services include annual exams, clinical breast exams, birth control, and prenatal care. Our providers also coordinate with local OB’s for delivery.

Walk-ins are accepted, but appointments are preferred.
